Manejo nutricional en un paciente con resección de íleon distal y fístula diafragmática / Nutritional management of a patient with resection of distal ileum and diaphragmatic fistula

RESUMEN

Paciente masculino de 35 arios de edad, fue impactado con 6 balas que afectaron la zona dorsolumbar, principalmente pulm6n, hígado, parte de intestino delgado (íleon distal) e intestino grueso (colon ascendente); es intervenido quirúrgicamente el 24/07/12 con un diagn6stico preoperatorio de trauma abdominal abierto, en el cual le realizaron una laparotomía exploratoria + resecci6n intestinal por necrosis de 2 m de íleon distal y colon ascendente con la colocaci6n de ileostomía y tubo de drenaje torácico concluyendo con un diagn6stico postoperatorio de peritonitis generalizada por necrosis intestinal, trauma diafragmático grado II por PAF, trauma hepático grado III por PAF y hematoma hepático. Posteriormente es intervenido nuevamente por presentar Absceso Pulmonar y hepático, tras la cual aparece una fistula diafragmática (fistula biliocutanea). ABSTRACT

Male patient of 35 years old, was struck with 6 bullets that hit the dorsolumbar area, mainly lung, liver, and part of small intestine (ileum) and large intestine (ascending colon); he is surgically treated the 24/07/12 with a preoperative diagnosis of open abdominal trauma, in which made an exploratory laparotomy + intestinal resection by necrosis of 2 meters of distal ileum and ascending colon with the placement of ileostomy and chest tube drainage; concluding in a postoperative diagnostic of generalized peritonitis by intestinal necrosis, diaphragmatic grade II trauma by PAF, hepatic trauma grade III by PAF and hepatic hematoma. Later, it is again surgically treated by presenting Lung abscess and liver, after which appears a diaphragmatic fistula (fistula biliocutanea).
