Influência da perda de ancoragem sobre o posicionamento final dos incisivos superiores e ANL / Influence of anchorage loss on the final positioning of upper incisors and nasolabial angle

RESUMO

As alterações no perfil facial produzidas pelas exodontias depré-molares é um assunto controverso na literatura ortodôntica. Dessaforma, o objetivo deste trabalho foi avaliar a perda de ancoragem e oposicionamento incisal ao final do tratamento, além de correlacionara posição final dos incisivos superiores com a perda de ancoragem,o lábio superior e o ANL, e dos incisivos inferiores com ambos os lábios.Foram mensuradas telerradiografias pré e pós-tratamento de 40pacientes adultos, de ambos os gêneros, com má-oclusão de Classe 11,divisão primeira, subdivididos em dois grupos de 20, de acordo com apresença ou não de apinhamento severo no arco inferior. Grupo 1 exodontiade quatro pré-molares; Grupo 2 exodontia de dois pré-molares(superiores), subdivididos novamente em dois outros grupos de dez,tratados com braquetes de Alexander e Edgewise. Análise estatisticaatravés do teste t de Student e correlação de Pearson (95%) foramrealizadas. Verificou-se perda de ancoragem em todas as amostras,além de retração de corpo significativa para os grupos 1A, 1B e 2A,demonstrando a importância do controle de torque nos dentes anteriores.Além disso, foi observado que quanto maior a perda de ancoragem,maior a protrusão do lábio superior (grupos 1B e 2A); quanto maior aretração do incisivo, maior a retrusão do lábio (grupos 1B e 2B). Nãose pôde estabelecer correlação estatisticamente significativa entre aposição final do lábio superior e o ANL, sequer entre a posição dosincisivos inferiores e os lábios superior e inferior.

ABSTRACT

Changes in facial proiile produced by the premolarextractions are controversial in orthodontic literature. Thus, the aimof this study was is to verify anchorage loss and incisal positioning atthe end of treatment, and to correia te the final positioning of the upperincisors with the upper lip and ANL, and of lower incisors with both lips,using two different brackets (Edgewise and Alexander) in premolarextractions cases. The sample consisted in 40 adult patients, of bothgenders, with Class 11, first division malocclusion, subdivided into twogroups of 20, according to the presence or absence of severe crowdingin mandibular arch (Group 1 four premolar extraction/Group 2 twoupper premolars extraction), subdivided again in two other groups often, treated with Alexander or Edgewise brackets. Statistical analysis byStudent's t test and Pearson correlation (95%) were performed. It wasfound loss of anchorage in ali samples, and body retraction signiticantfor the groups 1A, 1B and 2A showing the importance of controllingtorque in anterior teeth. Furthermore, it was observed that the higherthe loss of anchorage, the higher the protrusion of the upper lip (groups1B and 2A), the higher the shrinkage of the incisor, the greater theretraction of the lip (groups 1B and 2B). We were unable to establisha statistically significant correlation between the final position of theupper lip and ANL neither between lower incisors position and upperand lower lips.
