Comparação cefalométrica de casos tratados com extrações de primeiros pré-molares e molares / Cephalometric comparison of cases treated with extraction of first premolars or molars

RESUMO

Objetivo: o objetivo deste trabalho foi comparar as alterações cefalométricas em casos tratados com extração dos quatro primeiros pré-molares e quatro primeiros molares permanentes. Metodologia: a amostra retrospectiva consistiu de 36 indivíduos divididos em dois grupos G1 25 pacientes com idade média inicial de 14,61 anos, tratados ortodonticamente com extrações dos quatro primeiros pré-molares; G2 11 pacientes com idade média inicial de 21,57 anos, tratados ortodonticamente com extrações dos quatro primeiros molares permanentes. A comparação dos grupos foi realizada a partir de telerradiografias em norma lateral iniciais e finais, e modelos de estudo iniciais de cada um dos indivíduos. Para a comparação intergrupos, foi utilizado o teste t independente. Resultados: com o tratamento, houve maior aumento do comprimento efetivo da maxila no G2, quando comparado ao G1. As alterações foram semelhantes no G1 e G2 para o componente mandibular, relação maxilomandibular e perfil tegumentar. Houve rotação do plano oclusal no sentido anti-horário no G1, e no sentido horário no G2. Os incisivos superiores foram mais retraídos, verticalizados e extruídos, e os molares superiores mais mesializados no G2. Os incisivos inferiores foram significantemente mais verticalizados no G1, e os segundos molares inferiores foram mais mesializados no G2. O trespasse vertical foi diminuído no G1 e aumentado no G2, e houve maior correção da relação molar no G1. Conclusão: casos com extrações de molares mostraram maior retração e verticalização dos incisivos superiores, porém, com a mesma alteração no perfil tegumentar, quando comparados a casos tratados com extrações de pré-molares.

ABSTRACT

Objective: the aim of this study was to compare the cephalometric changes between cases treated with extraction of four first premolars and four first permanent molars. Methodology: The retrospective sample comprised 36 subjects divided into two groups G1 25 patients with a mean initial age of 14.61 years, treated orthodontically with extraction of four first premolars; G2 11 patients with a mean initial age of 21.57 years, treated orthodontically with extraction of four first permanent molars. The comparison between groups was made from lateral cephalograms from the initial and final stages and dental casts from initial stage of each subject. For intergroup comparison, independent t tests were used. Results: with treatment, there was a greater increase in maxillary effective length in G2, when compared to G1. Changes were similar in G1 and G2 for the mandibular component, maxillomandibular relationship and tegumental profile. There was a counterclockwise rotation of the occlusal plane in G1 and in clockwise rotation in G2. The maxillary incisors were more retracted, uprighted and extruded and maxillary molars were more moved mesially in G2. Mandibular incisors were significantly more uprighted in G1 and second mandibular molars were more moved mesially in G2. The overbite was decreased in G1 and increased in G2, and there was more correction of the molar relationship in G1. Conclusion: cases treated with molars extraction showed more retraction and uprighting of maxillary incisors, however with same change in tegumental profile when compared to cases treated with premolars extraction.
