Atención primaria fortalecida como principal ingreso al sistema de salud argentino / Strengthening primary care as main point of entry to the Argentine health system

RESUMEN

Contexto: el sistema de salud argentino es un sistema fragmentado, de cuidado episódico, con un nivel de gasto importante e indicadores que deberían seguir mejorando. Lo integran tres subsistemas público, de seguridad social y privado. A pesar de los esfuerzos de estos subsistemas, los pacientes no perciben a la atención primaria de salud como un acceso válido a la atención sanitaria, por lo que prefieren escoger en forma anárquica desde una cartilla de prestadores. Objetivo: describir y replantear desafíos para que la atención primaria de salud se consolide como la vía de ingreso principal del sistema de salud argentino. Análisis en la actualidad se ha documentado la magnitud de inequidades en salud, en que las diferencias de vulnerabilidad y exposición contribuyen a agudizar la estratificación social. Esta situación hace necesario organizar una estrategia coordinada, efectiva y sostenible que permita resolver los problemas de salud existentes, afrontar nuevos desafíos y mejorar la equidad. Conclusión: la atención primaria de salud debe convertirse en el principal ingreso al sistema sanitario argentino. Es un camino largo que exige decisiones conjuntas y coordinadas, orientadas a fortalecer un modelo de atención basado en la prevención y detección precoz de enfermedades, generando una asignación eficiente del gasto y una mejora en la calidad de vida de la población.

ABSTRACT

Background. The Argentine health system is characterized by fragmentation of care, major spending levels and indicators that beg to improve. It is composed of three subsystems public, private and social security. Despite the efforts of these subsystems, patients do not perceive primary health care as a valid point of entry to health care, preferring to haphazardly choose from a roster of providers. Purpose. To describe and reframe the challenges facing primary health care in order to become the main point of entry to the Argentine health system. Analysis. The magnitude of health inequities have been amply reported, wherein differences in vulnerability and exposure to disease lead to greater social stratification. This situation makes it necessary to organize a coordinated, effective and sustainable strategy to tackle existing health problems, new challenges and improve equity. Conclusion. Primary health care should become the main point of entry to the Argentine health system. This is a long term endeavor that requires joint and coordinated decisions aimed at strengthening a model of care based on prevention and early detection of disease, together with efficient spending and an improvement in the quality of life of the population.
