Cuidado cultural del diabético amputado / Cultural care for the diabetic amputee

RESUMEN

ObjetivoIdentificar el cuidado émic y el cuidado étic documentado en pacientes con amputación por pie diabético con el fin de plantear los modos de acción-decisión que faciliten un cuidado de Enfermería culturalmente congruente en el hogar de estos pacientes.MetodologíaPara la descripción del cuidado émic se usaron como técnicas de recolección de información la observación, las notas de campo y las entrevistas abiertas y a profundidad. Se aplicó la matriz del marco lógico y los constructos de cuidado émic, étic y modos de acción-decisión de la teoría de Madeleine Leininger de la diversidad y universalidad de los cuidados culturales. ResultadosSe encontraron prácticas a mantener como adaptación de lugares de la casa para el aseo propio, bolsas bajo el muñón, uso de caléndula, retiro de prótesis para dormir, alimentación hipoglúcida, apoyo familiar y colocación de imágenes religiosas. Los cuidados a reestructurar fueron uso de cortauñas y uso de la planta cordoncillo y los cuidados a negociar los relacionados con el uso de talco, la permanencia en lugares con moscas, la regulación de la adherencia farmacológica y el uso de hierbamora.ConclusionesSe encontró que la mitad de los cuidados identificados en el hogar deben negociarse y reestructurarse para poder brindar cuidado culturalmente congruente y la otra mitad deben mantenerse fomentando su preservación.

ABSTRACT

ObjectiveTo identify the emic and etic care documented in patients with diabetic foot amputation in order to raise the modes of action-decision to facilitate a culturally congruent nursing care at home for these patients.MethodologyFor the description of the emic care, the following data collection techniques were used observation, field notes, and open and in-depth interviews. It was applied the logical framework matrix and the constructs of emic and etic care and modes of action-decision from Madeleine Leininger's theory of diversity and universality of cultural care.ResultsSome practices that should be kept were found, such as adaptation of places in the house for their own personal cleaning, bags under the stump, use of calendula, removal of prosthesis before sleeping, low-sugar food, family support, and placement of religious images. The cares to be restructured were the use of nail clippers and the use of the Cordoncillo plant. The ones to be negotiated were the use of talcum, staying in places with flies, the regulation of medication adherence and the use of nightshade.ConclusionsIt was found that half of the identified home care should be negotiated and restructured to provide culturally congruent care, and the other half should be kept encouraging their preservation.
