RESUMO
O tratamento ortodôntico-cirúrgico das más oclusões do Padrão III visa corrigir não somente a oclusão, mas, especialmente, a desarmonia facial. Com essa perspectiva, o diagnóstico deverá priorizar a face, identificando a localização e gravidade do problema, de acordo com o gênero, idade, cor e, o mais importante, dando ênfase à queixa principal do paciente. O tratamento ortodôntico descompensatório deve criar relações oclusais condizentes com a desarmonia facial e oferecer ao cirurgião a chance de mudar, tanto quanto o necessário, as relações faciais. O presente trabalho tem por objetivo ilustrar, por meio de um caso clínico, a sequência de tratamento de uma paciente adulta, assimétrica, Padrão III por deficiência maxilar, com queixa principal relacionada à moderada desarmonia facial. O tratamento envolveu ortodontia descompensatória e cirurgia ortognática de avanço maxilar, resgatando a condição de normalidade oclusal e facial, atendendo plenamente a queixa da paciente.
ABSTRACT
Orthodontic-surgical treatment of Pattern III malocclusions aims to correct not only the occlusion, but specially to achieve the facial harmony. Thus, the diagnosis should prioritize facial aesthetics, identifying the localization and the severity of the problem, according to the gender, age, race and the patient main complaint. The decompensation orthodontic treatment aims to create an occlusal condition compatible to the facial disharmony offering the surgeon the possibility to perform changes during the correction of facial relation. The present article aimed to illustrate by a case report, the treatment sequence of an adult Pattern III patient with maxillary deficiency who presented as the main complaint was facial disharmony despite a mild facial discrepancy. The treatment involved decompensation orthodontics and orthognathic surgery for maxillary advancement, recovering the normal occlusion and facial condition.
