RESUMEN

Introducción:

La infección por VIH SIDA requiere tratamiento crónico al ser hasta la fecha incurable, siendo un objetivo primordial de la terapia alcanzar y mantener la supresión viral para disminuir la mortalidad, morbilidad, la transmisión del virus y la emergencia de cepas virales resistentes a los antiretrovirales. Los países que han organizado programas de tratamiento, realizan constantes esfuerzos para su financiamiento. El problema Nuestro país no ha implementado estrategias para su medición sistemática y su sostenimiento.

Justificación:

El conocimiento de la frecuencia y características de la supresión viral en la cohorte de pacientes peruanos beneficiara el funcionamiento del programa nacional al permitir evaluar su eficacia para mejorar la intervención. Objetivo General Analizar la duración de la SVS en pacientes adultos con VIH/SIDA tratados con antiretrovirales desde el periodo 2004-2006 hasta el 2012 en el Programa Nacional Peruano.

Métodos:

Estudio observacional retrospectivo en la cohorte de datos abiertos de adultos con VIH/SIDA, seguidos hasta el 31 de diciembre del 2012. Se analizó la presencia y duración e la supresión viral sostenida y los factores relacionados. Análisis Análisis descriptivo de datos demográficos, indicadores clínicos basales y de seguimiento de pacientes RESP y no-RESP. Análisis Bivariado comparando pacientes RESP con y sin FSV. Análisis de supervivencia (Kaplan-Meier) en RESP para estudiar la duración de la SV y su relación con variables independientes. Los hallazgos fueron evaluados mediante el análisis de riesgos proporcionales de Cox y análisis de residuos de Schoenfeld. Se utilizó software R, versión 3.0.3., paquete survival...
ABSTRACT

Introduction:

HIV AIDS is an incurable disease and requires chronic treatment, with a primary goal of therapy to achieve and maintain viral suppression to reduce mortality, morbidity, virus transmission and the emergence of resistant viral strains to antiretroviral. Countries that have organized treatment programs make constant efforts for funding.The

problem:

Our country has not implemented strategies for systematic measurement and its support over time.Justification Knowledge of the frequency and characteristics of viral suppression in the cohort of Peruvian patients will strength the capabilities of the national program to assess its effectiveness to improve current health interventions.General

Objective:

To analyze the duration of sustained viral suppression in adult HIV/AIDS patients treated with antiretroviral commencing 2004-2006 and followed until 2012 in the Peruvian National Program.

Methods:

Retrospective observational study in an open-data cohort of adult HIV/AIDS patients followed until December 31, 2012. The presence and duration of sustained viral suppression and related factors were analyzed.

Analysis:

Descriptive analysis of demographic data and clinical indicators at baseline and follow-up of responders (RESP) and non-responders (non-RESP) cases. Bivariate analysis RESP cases with and without viral suppression failure was also performed. Survival analysis (Kaplan-Meier) was applied in RESP to study the duration of viral suppression and its relationship with the independent variables. The findings were evaluated by analysis of Cox proportional hazards and Schoenfeld residuals. R Software was used, version 3.0.3, survival package.

Results:

A total of 6289 patients were studied. 1147 were only screened and did not continue with the program, 612 initiated treatment but never achieved viral suppression (non-RESP) and 4530 achieved viral suppression (RESP) and held this by variable time...
