A pesquisa analisou, no período de 2001-2008, a composição público-privada na rede municipal de saúde e aspectos do processo de contratualização dos serviços. O método de investigação foi o estudo de caso com pesquisa documental e entrevistas. Os sujeitos entrevistados foram ex-secretários de saúde, diretores da regulação e gerentes distritais. As categorias de análise utilizadas foram fundo público, redes de atenção e controle público. Os resultados demonstraram que a contratualização se restringiu a unidades filantrópicas. Em relação aos demais estabelecimentos privados vinculados à rede pública constatou-se o não cumprimento de aspectos programáticos, tais como a regulamentação dos processos licitatórios necessários para a contratualização. As instâncias gestoras não atuaram na direção de fortalecer os serviços públicos estatais, tampouco na formação de redes de atenção. Os estabelecimentos contratualizados desenvolveram suas atividades sem os efetivos mecanismos de controle, externo e interno, imprescindíveis ao uso adequado dos recursos que são públicos. As autoras concluem que a contratualização não alterou, significativamente, o padrão de compra e venda de serviços, não se constituindo, de fato, em um processo fortalecedor do papel da esfera pública.
ABSTRACT
This research analyzed the public-private composition in the municipal health network and aspects of the contracting/outsourcing process for services over the period from 2001 to 2008. The research method used was a case study with documentary research and interviews. The interviewees were former secretaries of health, directors of regulation and district managers. The categories of analysis used were public funds, care networks and public control. The results showed that the contracting was restricted to philanthropic units. With respect to the other private establishments linked to the public care network, non-compliance with programmatic aspects was detected, such as the lack of regulation of bidding processes required for contracting. Management authorities did not actively pursue building up state public services, or the formation of care networks. The contracted establishments conducted their activities without effective external and internal control mechanisms, which are paramount for the proper use of public resources. The authors conclude that the contracting process does not significantly alter the standard of buying and selling of services and indeed does not enhance the empowering process of the role of the public domain.
