Fatores predisponentes para lesão renal aguda em pacientes em estado crítico: revisão integrativa / Factors predisposing for acute kidney injury in patients in critical condition: integrative review

RESUMO

JUSTIFICATIVA E OBJETIVOS: Consideramos que a identificação dos fatores desencadeantes e/ou predisponentes da lesão renal é de grande relevância para sua prevenção e controle; e também para estudos futuros. Portanto, este estudo tem como objetivo realizar uma análise crítica das evidências disponíveis referentes ao tema "fatores predisponentes para lesão renal aguda em pacientes em estado crítico". CONTEÚDO: O presente estudo constitui-se em uma revisão bibliográfica integrativa e foi elaborado conforme as seis fases descritas por Ganong (1987). Foram incluídos artigos da base de dados Medline/Pubmed publicados nos últimos cinco anos, encontrados na língua inglesa e portuguesa. Foram selecionados 42 artigos, sendo que 35 artigos contemplavam informações a respeito do público adulto e os sete restantes sobre fatores predisponentes para lesão renal aguda em neonatos e crianças; que posteriormente foram agrupados em quatro categorias. Os resultados podem ter sofrido alguma influência decorrente das inúmeras nomenclaturas e critérios diagnósticos. Sepse e medicamentos nefrotóxicos foram os principais fatores predisponentes encontrados nos artigos incluídos neste estudo; merece destaque também cirurgias, principalmente cardíaca, hipovolemia e hemotransfusão. CONCLUSÃO: Diante disto podemos avaliar que existe uma variedade de fatores que predispõe à lesão renal aguda, a qual acarreta uma cascata de problemas: sofrimento para o paciente, internação hospitalar prolongada, aumento de custos hospitalares, necessidade de Terapia de Substituição Renal, podendo decorrer em Doença Renal Crônica ou até mesmo a morte.

ABSTRACT

BACKGROUND AND OBJECTIVES: We believe that the identification of inducing and/or predisposing factors for renal injury is of the great relevance for your prevention and control; and also for future studies. Therefore, this study aims to realize a critical analysis of available evidence on the topic "predisposing factors for acute kidney injury in critically ill patients." CONTENT: This study consists in an integrative literature review and were developed the six phases described by Ganong (1987). Articles in Medline/Pubmed published in the last five years found in English and Portuguese were included. 42 articles were selected, of which 35 articles contemplated information about the adult public and the remaining seven on predisposing factors for acute kidney injury in neonates and infants; which were subsequently grouped into four categories. The results may have been suffering some influence from the numerous classifications and diagnostic criteria. Sepsis and nephrotoxic drugs were the main predisposing factors found in the articles included in this study; also noteworthy the surgeries, especially heart, blood transfusion and hypovolemia. CONCLUSION: In this view we evaluate that there are a variety of factors that predispose to acute renal injury, which causes a cascade of problems: suffering for the patient, prolonged hospitalization, increased hospital costs, need for Renal Replacement Therapy that could result in Chronic Kidney Disease or even death.
