RESUMEN
La vitamina D es una hormona, debido a que los organismos vivos la sintetizan por exposición solar. Este mecanismo es su principal fuente, mediante la conversión por UVB en la piel del dehidrocolesterol a previtamina D3, la que rápidamente pasa a vitamina D3 (colecalciferol). En su primera hidroxilación enzimática, en el hígado, el colecalciferol es convertido en 25-hidroxivitamina D en la segunda hidroxilación, en el riñon, la 25-hidroxivitamina D es convertida en 1,25-hidroxivitamina D (calcitriol). Es su forma activa y es regulada por la paratohormona, la concentración de calcio y fósforo. La segunda fuente de vitamina D es la dieta y los suplementos vitamínicos. La vitamina D tiene múltiples y diferentes funciones, entre ellas las siguientes la homeostasis de calcio por acción del riñon, los huesos y el intestino; la regulación de las poblaciones celulares linfocitarias; acción antioxidante; regulación del sistema reninca-angiotensina-aldosterona; producción de insulina. De esto se desprende su utilidad en enfermedades donde interviene el mecanismo de inmunidad, como en la psoriasis, dermatitis autópica esclerodermia, lupus eritematoso sistémico.
ABSTRACT
Vitamin D is considered a hormone because living organisms synthesize it by sun exposure; being by this mechanism the main source through conversion by UVB between 290 and 315 nm in skin dehydrocholesterol to previtamin D3 quickly passes vitamin D3 known as cholecalciferol, then the first step by enzymatic hydroxylation in the liver is converted to 25, hydroxyvitamin D and the second step is in the kidney by another hydroxylase is converted into vitamin D or calcitriol 1.25 is its active form being regulated by the parathyroid hormone, calcium concentration and phosphor. La second source of vitamin D is diet and vitamin supplements. Vitamin D has many different functions including calcium homeostasis per share in kidney, bone, intestine; in the regulation of cell growth and in this case differentiation keratinocytes; involved in regulating lymphocyte immune cell populations, acts as an antioxidant; regulating the renin, angiotensin, aldosterone also insulin production system. It a appears useful in diseases where immune mechanism intervenes as psoriasis, atopic dermatitis, scleroderma, systemic lupus erythematosus. Since 1994 when it was approved by the FDA for topical calcitriol psoriasis, have appeared similar with fewer side effects and better effectiveness including calcipotriol and tacalcitol.
