Este estudo descreve o itinerário terapêutico de pacientes assistidos no Instituto Benjamin Constant (IBC) identificando as barreiras de acesso a serviços de saúde especializados em oftalmologia no SUS. Buscamos descrever e compreender de que modo os obstáculos presentes nos serviços de saúde impedem o acesso à assistência oftalmológica.Informações levantadas sobre a assistência oftalmológica, documentos oficiais, as narrativas dos pacientes atendidos no IBC, e de alguns funcionários, foram utilizados a fim de compreender a lógica da assistência oftalmológica no serviço de saúde. Para o reconhecimento dos itinerários percorridos, dezenove pacientes foram entrevistados, também foram realizadas duas entrevistas com profissionais da instituição. Optamos pelo IBC como campo de estudo para a realização das entrevistas, pois a instituição é uma tradicional escola para deficientes visuais, e apesar de ser vinculada ao Ministério da Educação, ela também oferece assistência oftalmológica à comunidade. Ao longo da pesquisa, percebemos lógicas diferentes reveladoras de não haver uma integração efetiva na rede de atenção estadual. Os serviços oftalmológicos disponíveis na rede de atenção do SUS não estiveram acessíveis para os entrevistados que recorreram a serviços privados para terem acesso sendo onerados com os altos custos dos tratamentos. Quando o acesso é alcançado, não existe a integralidade na assistência, resultando em pacientes desorientados a procurar por assistência oftalmológica.
ABSTRACT
This study describes the therapeutic itinerary of patients assisted the Benjamin Constant Institute (IBC) identifying the access to health care barriers specialized in ophthalmology inSUS. We seek to describe and understand how the present obstacles in health services prevent access to eye care.Raised information about eye care, official documents, the narratives of patients seen at IBC,and some employees were used in order to understand the logic of eye care in the health service. For the recognition of therapeutic itineraries, nineteen patients were interviewed, were also held two interviews with professionals of the institution. We opted for IBC as field of study for the interviews because the institution is a traditional school for the visually impaired, and despite being under the Ministry of Education, it also provides eye care to thecommunity. During the research, we realized different logical revealing that there is no effective integration in the state care system. Eye care services available in the SUS are not accessible to respondents who resorted to private services for access being burdened with the high costs of treatment. When access is achieved, there is no integral care, resulting in patients disoriented looking for eye care.The construction of the therapeutic itinerary in search of attention, through the IBC patients reports, allowed accumulating reflections on access ophthalmic assistance in health carenet works in Rio de Janeiro state. Thus, the results indicate the need for expansion and organization of the network of attention to eye health is a fundamental aspect of comprehensive care by facilitating access to care. The expansion of public policies that prioritize prevention and promotion of eye health in primary care should also be considered, contributing to preventive care vision.
