Papel de la artroscopia en las lesiones traumáticas del tobillo / The role of arthroscopy in traumatic lesions of the ankle

RESUMEN

Fundamento las lesiones traumáticas alrededor de la articulación del tobillo son frecuentes y aunque se logra la reducción anatómica, en muchas ocasiones el resultado no es el esperado. Las lesiones intrarticulares indetectables a simple vista son las responsables de los resultados de estos tratamientos. Para este tipo de lesiones la artroscopia ofrece ventajas diagnósticas y terapéuticas. Objetivo: realizar una revisión sobre el tratamiento artroscópico de pacientes con lesiones traumáticas del tobillo. Métodos: se realizó una revisión bibliográfica de un total de 330 artículos publicados en Pubmed, Hinari y Medline mediante el localizador de información Endnote. Se utilizaron 50 citas seleccionadas para realizar la revisión, 44 de ellas de los últimos cinco años y se incluyeron cuatro libros. Desarrollo se analizaron las ventajas de la artroscopia en la articulación del tobillo en las lesiones traumáticas, sus indicaciones y contraindicaciones. Se describió la técnica quirúrgica a emplear apoyado en dos imágenes propias del autor. Se abordaron las enfermedades traumáticas que más inciden en la articulación, así como sus peculiaridades. Conclusiones: la artroscopia como tratamiento complementario en lesiones traumáticas del tobillo ofrece ventajas al permitir el diagnóstico y tratamiento de lesiones intrarticulares que interfieren en el resultado funcional de los enfermos.

ABSTRACT

Background: traumatic lesions around the ankle joint are frequent and, in spite of the fact that the anatomical reduction is achieved, the result obtained is not sometimes the expected. Intra-articular lesions, undetectable at first sight, are the cause of the results of these treatments. Arthroscopy offers diagnostic and therapeutic advantages for this type of lesions. Objective: to make a review on the arthroscopic treatment of patients with traumatic lesions of the ankle. Methods: a bibliographic review of 330 articles published in Pubmed, Hinari and Medline was made by means of the reference management software Endnote. Fifty citations were selected to make the review, 44 of them were from the last five years; four books were included. Development the advantages of arthroscopy in the ankle joint to treat traumatic lesions, its indications and contraindications were analyzed. The surgical technique to use, based on two pictures by the author, was described. The traumatic diseases that most affect this joint were discussed, as well as its peculiarities. Conclusion: arthroscopy as a complementary treatment for traumatic lesions of the ankle offers advantages that allow the diagnosis and treatment of intra-articular lesions that interfere with the functional result of patients.
