RESUMEN
La obstetricia clásica se fundamenta en la atención pasiva durante el embarazo y la pesquisa de factores de riesgo mediante la historia clínica y exámenes de laboratorio con una habilidad predictiva limitada, además de una disposición activa al acercarse el parto y durante el trabajo de parto mismo. Así, los resultados se basan en criterios de morbilidad y mortalidad asociados al “evento quirúrgico” del parto. El desarrollo de la medicina fetal ha permitido un enfoque activo de pesquisa de riesgo materno, obstétrico y fetal, de modo de producir una selección o cribado poblacional de aquellos embarazos cuya progresión constituye un riesgo para la salud del binomio madre-hijo, redefiniendo aquellas condiciones médicas determinantes de consecuencias a corto y largo plazo en los recién nacidos, especialmente, la prematurez y la insuficiencia placentaria. ABSTRACT
Traditionally, Obstetrics is divided in two separated ways being mainly passive throughout pregnancy with risk selection based on medical history and basic laboratory test, which have limited prediction ability. Later on pregnancy as delivery is imminent and during labor, medical approach is getting more active and frequent. Therefore, classical obstetrics results are based on morbidity and mortality criteria related to the “chirurgical event” of delivery. Development of fetal medicine has focused on selection of maternal, obstetrical and fetal risk, thus generating a population-based screening program and recognizing those pregnancies in which progression could be related with risks for either the mother or the child and redefining medical conditions that determine short-term or long-term complications, mainly prematurity and placental failure.
