Osteonecrosis de los maxilares asociada al uso de bifosfonatos / Bisphosphonate-related osteonecrosis of the jaws

RESUMEN

Los bifosfonatos (BP) son fármacos análogos a la molécula del pirofosfato que inhiben la reabsorción ósea. Estos medicamentos son usados en la prevención y tratamiento de enfermedades que involucran procesos de reabsorción ósea, como la osteoporosis y algunas formas de cáncer. En 2003 el Dr. Robert Marx encendió la alarma mundial al comunicar 36 casos de pacientes con exposiciones óseas sintomáticas en sus maxilares, secundarias a procedimientos quirúrgicos bucales e incluso de aparición espontánea, estando todos estos enfermos bajo tratamiento con bifosfonatos, como zoledronato o pamidronato. Esto se explica porque los maxilares son los únicos huesos que tienen interacción con el ambiente mediante vía dentaria, siendo más propensos a sufrir alteraciones que otros huesos del cuerpo. Desde entonces han existido múltiples estudios respecto al desarrollo de osteonecrosis de los maxilares asociados al uso de bifosfonatos (BRONJ). ABSTRACT

Bisphosphonates (BP) are a group of drugs like phirosphate molecule that inhibit bone resorption. Bisphosphonates are commonly prescribed for prevention and treatment of disease whit bone loss caused by osteoporosis and some types of cancer. In 2003, Dr. Robert Marx, turned on the world alarm, reporting 36 diagnosed cases whit painful bone exposure in the mandible and maxilla, after bucal surgery treatment or spontaneously, all the patients receiving the biphosphonates pamidronato and zoledronato. The maxilla and mandible were the only bones involved in this exposure, because they are more exposed to enviroment throw de teeth, been more susceptible to disease than other bones of the body.
