Tratamiento quirúrgico de las fracturas del extremo proximal de fémur en niños: experiencia institucional / Surgical treatment of fractures of extreme proximal femur in children: institutional experience

RESUMEN

Las fracturas del extremo proximal del fémur por las características peculiares en su vascularización y la presencia del cartílago de crecimiento, para su tratamiento tengan un criterio bien establecido. En nuestro nosocomio representan el 1% de las lesiones fracturarias que se internan en el servicio de ortopedia y traumatología, que obedecen por lo general a traumas de alta energía, asociadas a politraumatismos (TCE, Trauma toráxico, abdominal y de pelvis) que implican gran riesgo de perder la vida. Por tanto consideramos que el tratamiento quirúrgico es una alternativa en la resolución de estas fracturas sin desmerecer las ventajas del tratamiento conservador, sin embargo las fuerzas biomecánicas alrededor de la cadera hacen que las fracturas consoliden en posiciones viciosas, por lo que decidimos en el servicio por un tratamiento quirúrgico de reducción cruenta y osteosíntesis como la mejor alternativa. Con la utilización de los tornillos canulados, los resultados son satisfactorios, la rehabilitación es pronta y se minimiza las complicaciones postoperatorias.

ABSTRACT

Fractures of the proximal end of the femur, due to the peculiar characteristics of their vascularization and the presence of growth cartilage, require well established criteria for treatment. In our hospital, they constitute 1% of the fractural lesions that are admitted to the section of orthopedics and traumatology and that correspond generally to high energy trauma associated with polytrauma (TCE, thoracic, abdominal, and pelvic trauma ) which implies a high risk of death. For this reason we consider that surgical treatment is an alternative for the resolution of these fractures without comparing unfavourably with the advantages of the conservative treatment. The biomechanical forces around the hip, however, cause the fractures to heal in bad positions. For this reason, in our section, we decided in favour of surgical treatment by a bloody reduction with osteosynthesis as the best alternative. With the use of cannulated screws, the results are satisfactory, rehabilitation is fast and post surgical complications are minimized.
