A história natural da instabilidade anterior do joelho por radiografia em estresse / The natural history of anterior knee instability by stress radiograph

RESUMO

Analisar o deslocamento anteroposterior do joelho pormeio de radiografias de estresse em indivíduos com instabilidadeunilateral anterior do joelho e correlacionar com o tempo de instabilidade.Métodos: Sessenta indivíduos com joelhos íntegros (grupocontrole) e 125 pacientes com instabilidade unilateral anterior dojoelho (grupo Al) concordaram em participar do estudo. Sexo,idade, peso, altura, idade por ocasião da lesão, tempo entre alesão e testes, e resultados cirúrgicos foram estudados. Ambos osgrupos foram submetidos a radiografias de stress anterior e posteriorde ambos os joelhos. Diferenças de deslocamento anterior(DDA) e posterior (DDP) foram calculadas. Resultados: No grupocontrole DDA e DDP são, em média, zero. No grupo AI, ADD é emmédia 9,8 mm e PDD 1,92 mm. Sexo, idade, peso, altura, idadeem trauma e presença de lesões de meniscos não interviram nosvalores da DDA e DDP. Lesões do menisco aumentaram com otempo. DDA e DDP não se correlacionaram com a presença ounão de com lesões meniscais. DDA e DDP mostraram correlaçãoentre sí e com o tempo de lesão. Conclusão: Há uma sub-luxaçãopermanente anterior do joelho lesionado que está relacionada coma quantidade de deslocamento anterior, que, por sua vez, aumentacom o tempo. ABSTRACT

To analyze the anteroposterior displacement of theknee by means of stress radiographs in individuals with unilateralanterior knee instability and correlate with time of instability.Methods: Sixty individuals with intact knees (control group) and125 patients with unilateral anterior instability (AI group) agreedto participate in the study. Gender, age, weight, height, age atinjury, time between injury and testing, and surgical findings werestudied. Both groups were submitted to anterior and posteriorstress radiographs of both knees. Anterior (ADD) and posteriordisplacement difference (PDD) were calculated between sides.Results: In the control group ADD and PDD are, in average, zero,whereas in AI group ADD averaged 9.8mm and PDD, 1.92mm.Gender, age, weight, height, age at trauma and presence of meniscallesions did not intervene in ADD and PDD values.
