O cisto dentígero é o segundo tipo de cisto odontogênico mais frequente, desenvolvendo-se a partir de uma alteração do epitélio reduzido do órgão do esmalte, onde ocorre acúmulo de líquido entre esse epitélio e o esmalte. Normalmente está relacionado à coroa de um dente não irrompido, unindo-se a ele na junção amelocementária. Desenvolvem-se assintomáticos, sendo geralmente descobertos em exames radiográficos de rotina ou ausência de erupção, apresentando-se radiograficamente como uma área radiolúcida bem circunscrita, porém podem chegar a grandes proporções, causando assimetria facial e dor. Existem várias formas de tratamento, sendo seu prognóstico bastante favorável. O presente trabalho tem como objetivo relatar um caso clínico em que foi utilizada a técnica da descompressão para tratamento de cisto de grandes proporções, envolvendo a maxila de uma criança de 10 anos de idade, que se apresentava inicialmente com grande assimetria facial na hemiface esquerda. O caso foi acompanhado clínico-radiograficamente por 9 meses, e, ao final da descompressão cística, houve a diminuição considerável do tamanho da lesão, reorientação do eixo de erupção dos dentes permanentes, sem a necessidade de remoção destes. A técnica da descompressão mostrou-se eficiente para o caso apresentado, tornando-se uma opção viável para tratamento de lesões císticas de grandes proporções envolvendo pacientes jovens.
ABSTRACT
The dentigerous cyst is the second most frequent odontogenic cyst, the development of which starts from an alteration in the reduced epithelium of the enamel organ, where a liquid accumulation occurs between the epithelium and the organ. Normally it is related to the crown of an unerupted tooth, adhering to it at the cementoenamel junction. It is generally asymptomatic and is usually discovered in routine radiographic examinations or in the absence of eruption. Its radiographic aspect is a well-circumscribed radiolucent area, but it may assume large proportions, causing facial asymmetry and pain. There are several forms of treatment, and its prognosis is quite favorable. The aim of the present article is to report a clinical case in which the decompression technique was used in the treatment of a major cyst in a 10-year-old child, who initially presented a large facial asymmetry on the left side of her face and a cystic lesion extending from the left side of the jaw to the orbital floor, with displacement of a number of permanent teeth.. The case was followed clinically and radiographically for 9 months and at the end of the cystic decompression there was a considerable decrease in the size of the lesion and a reorientation of the eruption axis of the permanent teeth with no need for their removal.
