Exostose frontal: uma opção de tratamento / Frontal exostosis: an option of treatment

RESUMO

Exostoses são crescimentos ósseo-benignos, com origem desconhecida, que acometem diversas regiões do esqueleto humano, dentre elas, a face. Sua exérese ocorre quando existe acometimento funcional, estético e cancerofobia por parte do paciente. Como diagnóstico diferencial, podem ser citados osteoma e esclerose óssea. No seguinte trabalho, descreve-se o relato clínico de uma jovem que procurou atendimento odontológico especializado com queixa de lesão nodular em região frontal, de aspecto pétreo, assintomático e crescimento progressivo. Foram realizados exames clínico, laboratorial e imaginológico que levaram à hipótese diagnóstica de exostose frontal. Procedemos com a cirurgia de exérese da lesão, sob anestesia local, sendo o material colhido enviado para exame anatomopatológico. Alguns autores acreditam que fatores ambientais e hereditários estejam associados ao desenvolvimento e à evolução das exostoses, porém, por ser uma variação da anatomia, sua exérese está indicada, apenas, quando comprometer a função, adaptação de próteses e (ou) estética. As etiologias das exostoses frontais e o fato de essas acometerem com maior frequência, ou unicamente, o gênero feminino ainda são desconhecidas, existindo apenas hipóteses. Entretanto, é justificável a sua exérese, visando ao perfeito convívio social da paciente.

ABSTRACT

Exostoses are benign bone growths of unknown origin, affecting various regions of the human skeleton, including the face. Their removal occurs when there is functional and aesthetic impairment. In the differential diagnosis osteoma and bone sclerosis may be mentioned. This paper describes a clinical case of a young woman who sought specialist dental treatment complaining of a nodular lesion in the frontal region, hard to the touch, asymptomatic and exhibiting progressive growth. Clinical, laboratory and imaging examinations were performed, leading to the hypothesis of a frontal exostosis. We opted for surgical excision under local anesthesia, after which the collected material was sent for pathological examination. Some authors believe that environmental and hereditary factors are associated with the development and progression of exostoses; however, as they constitute an anatomical variation, their removal is indicated only in the case of impairment of function, fitting of dentures or aesthetic damage. The etiology of frontal exostoses and the fact that they most frequently affect females is still unknown, for there are only hypotheses. However, their removal is justified in order to relieve the great social suffering experienced by the victims.
