Uso de la técnica de colocación de una cinta suburetral transobturatriz en la histerectomía vaginal como tratamiento de la incontinencia urinaria de esfuerzo femenina / Use of the technique of suburethral sling placement through the obturator holes in hysterectomy vaginal as complementary treatment of female stress urinary incontinence

RESUMEN

El presente estudio pretende describir la experiencia del Hospital Nacional Arzobispo Loayza (HNAL) con la técnica de colocación de una cinta suburetral transobturatriz (transobturatriz tape, TOT) en la histerectomía vaginal (HV) como tratamiento de la incontinencia urinaria de esfuerzo (IUE) respecto a los tiempos quirúrgicos y la frecuencia de complicaciones. Material y Métodos. Estudio de serie de casos que incluye a pacientes sometidos a tratamiento quirúrgico para IUE con la técnica de TOT, de abril de 2009 a febrero de 2010, en el servicio de Ginecología del departamento de Ginecología y Obstetricia del HNAL. Se realizó la técnica de TOT en las pacientes que tenían indicación de abordaje por vía vaginal. Se evaluaron características demográficas y clínicas, así como también el tiempo quirúrgico total, quirúrgico para IUE y tiempo de hospitalización posquirúrgico. Además, se estimó la frecuencia de complicaciones y episodios de IUE a las dos y cuatro semanas luego de la cirugía. Se utilizó el programa estadístico Statistical Package for Social Science 12.0. Resultados. El tiempo quirúrgico de la técnica TOT fue de 13,2 minutos. El tiempo quirúrgico total de la HV + TOT fue de 106,7 minutos, que es menor que el reportado en la literatura para otras técnicas operatorias. Ningún paciente reportó síntomas de IUE a las dos y cuatro semanas poscirugía. La frecuencia de complicaciones a las cuatro semanas de seguimiento fue menor de 4 % de los casos, principalmente infección urinaria y retención urinaria. Conclusión. La experiencia del HNAL con la técnica TOT en la HV como tratamiento de la IUE mostró menores tiempos quirúrgicos y una frecuencia de complicaciones menor de 4 %...

ABSTRACT

This study seeks to describe the experience of the Arzobispo Loayza Hospital with the TOT technique in vaginal hysterectomy as adjunctive treatment of stress urinary incontinence regarding the success rate and complications of that surgical technique. Material and Methods. This is a case series of patients who underwent surgical treatment for stress urinary incontinence using the TOT technique in the Gynecology Service at the Arzobispo Loayza Hospital from April 2009 to February 2010. This technique was applied to those patients who had indication of a vaginal approach. Demographic and clinical characteristics were assessed, as well as the total surgical time, surgical time for the SUI and time of the post-surgical hospitalization. In addition, the frequency of complications and SUI events were determined at the second and fourth weeks after the surgery. The Statistical Package for Social Science 12.0 (SPSS Inc., Chicago, IL. USA) was used to perform the analysis. Results and conclusion. The surgical time for the TOT technique was 13,2 minutes. The total surgical time of the HV + TOT was 106,7 minutes, which is less than the time reported for other techniques for the treatment of SUI. None of the patients experienced symptoms of SUI at the second and forth weeks after the surgery, which suggests a high level of success at that time of follow-up.
