Infarto posterior por embolia coronária em portador de prótese valvular: relato de caso / Posterior infarction due to coronary embolism in a patient with valve prosthesis. Report of a case

RESUMO

Paciente do sexo feminino, 56 anos, com disfunçäo mitral e fibrilaçäo atrial crônica, submetida a duas substituiçöes valvulares prévias. Portadora de hipercolesterolemia e näo usando anticoagulante, a mesma foi atendida em caráter de urgência com quadro clínico e eletrocardiográfico de infarto do miocárdio em parede posterior do ventrículo esquerdo, diagnóstico comprovado através de dosagem de CK-MB e cinecoronariografia. Os aspectos eletrovectocardiográficos encontravam identidade plena com aqueles descritos na literatura

ABSTRACT

A female patient, 56, with mitral heart disease and chronic atrial fibrilation is presented. She underwent mitral valve replacement with bioprosthesis twice previously. Suffering from hypercholesterolemia and not in anticoagulant therapy, she was admitted in the emergency room with clinical history and electrocardiogram of a posterior infarction. Diagnosis was later assured by serum CK-MB and coronary arteriography. The electrovectocardiographic aspects of the case agree with those reported in the literature.
