A CLINICAL STUDY OF ORTHOGNATHIC SURGERY PATIENTS' SATISFACTION AND PSYCHOLOGIC CHANGE

ABSTRACT
Jaw deformities are primarily a result of malrelation of the jaws that causes protrusion or retrusion of one or both jaw. The result is a dysfunction of the masticating mechanism with attendant disharmony of the facial appearance. Jaw deformities are the very displeasing esthetic, functional, and psychological problems to the patient and others. So, motivation for surgery include both functional and esthetic goals. In surgical correction of jaw deformities, patient satisfaction is a major goal of treatment. Despite of an esthetically acceptable outcome Patient dissatisfaction may occur due to poor communication between the patient and the surgeon. This is sometimes due to the lack of a clear understanding of the purpose of the operation by the patient and exactly what procedures are to be done. The purpose of this study is to examine functional, esthetic and overall satisfaction of the patients who received orthognathic surgery and pre and postoperative psychologic status and their change. The results were as follow 1. Motivation for surgery included both esthetic and functional goals. And patient's prime reason for surgery was esthetic correction of facial appearance(62%). 2. The satisfaction rate of orthognathic surgery was 81% during immediate postoperative period and 92% after 6 month follow-up period. The most important factor contributing dissatisfaction was postoperative sensory disturbance of facial soft tissue(50%) and residual deformities of jaw. 3. The most difficult factor during immediate postoperative period was difficulty in talking and eating by intermaxillary fixation. 4. The significant differences of personality characteristics were noticed in the Depression(D), Schizophreania(Sc), Hypomania(Ma) were changed significantly between immediate and 6-months after surgery.(ANOVA, p<0.05). 5. The significant differences of the self-concept scale were noticed in the B(Moral Ethcal Self Score), D(Family Self Score) were changed significantly following the orthognathic surgeries. (ANOVA, p<0.05).
