ABSTRACT
Objective To investigate the effects of dexmedetomidine on myocardial injury in adult liver transplantation.Methods Sixty patients of both sexes,aged 42-63 years,with body mass index (BMI) of 18-25 kg/m2,scheduled for adult liver transplantation,were randomly divided into two groups (n =30 each) using a random number tablecontrol group (group C) and dexrnedetomidine group (group D).In group D,dexmedetomidine was infused intravenously at a dose of 0.5μg/kg over 12 min immediately before skin incision,followed by 0.5 μg· kg-1· h-1 infusion until the end of surgery.The equal volume of normal saline was given in group C.Immediately before skin incision (baseline,T0),at 30 min of anhepatic phase (T1),at 30 min of neohepatic phase (T2),at the end of surgery (T3),at 4 h after surgery (T4) and at 24 h after surgery (Ts),blood samples were obtained from central vein for tumor necrosis factor-α (TNF-α),interleukin-6 (IL-6),high mobility family protein B1 (HMGB1),cardiac troponin I (cTnI),myoblobin (Myo) and creatine kinase isoenzyme (CK-MB).The occurrence of adverse cardiac events during and afer surgery for all patients was recorded.Results Compared with the baseline value at T0,the serum levels of TNF-α and IL-6 were increased at T1-5;The levels of HMGB1,cTnI,Myo and CK-MB were increased at T2-5 in both groups (P<0.05).Compared with group C,the serum concentrations of TNF-α and IL-6 were reduced at T1-5 in group D (P<0.05).the serum concentrations of HMGB1,cTnI,Myo and CK-MB were reduced at T2-5 in group D (P < 0.05).The occurrence of myocardial ischemia and supraventricular tachycardia was reduced at T1-5 in group D (P<0.05).Conclusion Dexmedetomidine can attenuate the myocardial injury of adult liver transplantation.
