ABSTRACT
Objective ToexplorethecomprehensivediagnosticvalueofMSCTand MRImultiphaseenhancedscaninsclerosing stromaltumorofovary(OSST),forimprovingtheaccuracyofpreoperativediagnosis.Methods CTand MRimagesof32patients withOSSTconfirmedbysurgicalandpathologicalfindingswereretrospectivelystudied.Theclinicalmanifestations,CTand MRI findingsofthetumorswereevaluated,andthecorrelationbetweentheimaging manifestationsandthepathologicalresultswasalso analyzed.Theclinicalandimagingmanifestationsofthetumorswereanalyzedandsummarized.Results Theageof32patientsranged from21to43yearsold.Allthemasseswereunilateral,15ontheleftand17ontheright.Themaximumdiameterofthelesionwas1.8cmto 19.5cm.AlllesionswereexaminedbyCTand MRI.MRIcouldbetterdisplaythecapsuleandstructureofthetumors.25caseswere solidandcysticmassesand7casesweresolid masses.Themarginofthetumorwasobviouslyenhancedinarterialphaseofmulti-phaseenhancedscanin28cases,andinthevenousanddelayedphases,theenhancementfeaturesweresimilartothoseof"hepaticcavernous hemangioma".Scatteredenhancednoduleswerefoundinthenon-enhancedcysticzone,showing"lakeislandsign".Inarterialphase,4 casesshowedearlymildandmoderateenhancementinthemarginofthelesion,andtheenhancementdegreeinvenousphaseanddelayedphase weresimilar,withoutobviouswithdrawalmediumofcontrast.TheCTnetvalueinthemassinarterialandvenousphaseweremoreobviousthan thatinthemyometriumandthedifferenceswerestatisticallysignificant,howevertherewasnosignificantdifferenceindelayedphase. Histopathologicalexaminationshowedthatthehighandlowcelldensityareaswereinterlacedwithpseudolobularstructure.Thereweredifferent amountofthecollagenfibers,fibrocyteandlooseedemaconnective tissue,aswellasabundantparenchymavesselsintheinterstitial tissue.ImmunohistochemicalstainingofVimentin,SMAandα-inhibinin32caseswereshowed,andPRexpressionswerefound in28cases.Conclusion OSSTischaracterizedby"lakeislandsign"onCTandMRI."Cavernoushemangioma"-likeenhancedfeaturecanbemore clearlydemonstratedbymulti-phasecontrast-enhancedscanning.Theseimagingfindingsarecloselyrelatedtothepathologicalfindings.The combineddiagnosisofCTand MRIimaginghasanimportantvalueforpreoperativediagnosis.
