Influences of 321 health education model on pruritus status of maintenance hemodialysis patients / 中国实用护理杂志

ABSTRACT

Objective:

To explore the nursing efficacy of 321 health education model on maintenance hemodialysis patients with itchy skin.

Methods:

A total of 97 cases of maintenance hemodialysis patients with itchy skin from January 2019 to December 2019 in the Central Hospital of Panzhihua were divided into control group of 49 cases and study group of 48 cases by table of random number. The patients in control group were taken with conventional nursing and health propaganda measures while the patients in study group accepted a different nursing mode of combining 321 health education model. The Chinese version of the Four-Item Itch Questionnaire (FIIQ), the Chinese version of the Dermatology Life Quality Index (DLQI), and the Chinese version of the Kidney Disease Questionnaire (KDQ) were used to evaluate the skin itchy status and its influences to patientslife and life quality of the 2 groups before and after intervention.

Results:

There was no significant difference in the skin itchy status and its influences to patientslife and life quality before intervention between the two groups( P>0.05). After intervention, the total score of FIIQ, and the scores of the three dimensions of itchy degree, itchy frequency, and influences on sleep were respectively (7.46 ± 0.94), (1.83 ± 0.26), (1.92 ± 0.29), (1.86 ± 0.23), which were all lower than the same data of the control group patients (12.36 ± 1.24), (2.69 ± 0.31), (3.02 ± 0.42), (2.91 ± 0.37), showing differences with statistical significance ( t values were 14.79-21.90, P<0.05). After intervention, the total score of the DLQI, and the scores of five dimensions of symptom perception, daily life, entertainment and leisure, work and study, and treatment of the patients in study group were respectively (10.55 ± 1.27), (2.85 ± 0.29), (2.11 ± 0.32), (2.47 ± 0.29), (1.72 ± 0.25), and (2.21 ± 0.27), which were all lower than the same data of patients in control group (14.47 ± 1.78), (3.92 ± 0.37), (1.57 ± 0.22), (1.64 ± 0.25), (1.26 ± 0.18), (1.21 ± 0.11), indicating statistically significant difference ( t values were 9.67-23.80, P<0.05). After intervention, the total score of KDQ and the scores of the four dimensions of physical symptoms, fatigue, depression, and frustration of the patients in study group were respectively (121.66 ± 15.91), (27.36 ± 3.02), (24.44 ± 3.44), (26.71 ± 3.58), and (19.28 ± 2.66), which were all higher than the same data of patients in control group (108.38 ± 14.69), (24.51 ± 2.64), (22.60 ± 3.41), (22.45 ± 3.47), (14.92 ± 2.04), presenting differences with statistical significance ( t values were -9.07 - -2.65, P<0.05).

Conclusions:

For maintenance hemodialysis patients with itchy skin, the nursing intervention by 321 health education model can effectively ease the itchy skin problem, relieve the impact of itchy skin on patients' lives, and improve the life quality of patients.
