Gigantomastia can cause physical and/or psychological discomfort. The conventional surgical treatment is usually reduction mammoplasty using the amputation technique and free nipple-areola grafting. This series included three large sizes women with this complain. A modification of the traditional vertical bipedicle reduction mammoplasty was used to preserve nipple sensation and maintain the physiological breast function; and at the same time to resect the required amount of breast tissue. The postoperative breast size was larger than the standard cup size breast resulted in other conventional methods; therefore, this technique was convenient for the large body sizes women. No serious postoperative complication was reported
