Abordagem de pacientes portadores de osteoradionecrose mandibular após radioterapia de cabeça e pescoço / Management of patients with osteoradionecrosis of the after radiation therapy to the head and neck

RESUMO

Introdução: O tratamento dos pacientes com osteoradionecrose mandibular é desafiador e quase sempre seguido de complicações. Objetivo: Descrever o tratamento de pacientes portadores de osteoradionecrose mandibular baseado no protocolo estabelecido por Marx e suas complicações. Método: Estudo retrospectivo de cinco pacientes com osteoradionecrose mandibular pós-tratamento radioterápico de cabeça e pescoço, tratados cirurgicamente no Hospital Santa Catarina de Blumenau/SC entre 2004 e 2007. Resultados: Um caso foi submetido à mandibulectomia seguida de reconstrução rígida associada à enxertia ilíaca, três submetidos á mandibulectomia segmentar com reconstrução rígida sem enxertia e um à mandibulectomia marginal. Em dois casos houve fechamento da ferida intrabucal e ausência de sintomatologia dolorosa. Nos três casos onde se realizou maior ressecção sem enxertia houve abertura da ferida cutânea com exposição da placa de reconstrução e necessidade de encaminhamento para cirurgia de reconstrução microvascular. Conclusão: O objetivo do tratamento da osteoradionecrose craniofacial é a ablação dos sintomas, da possibilidade da recorrência do tumor e restauração da forma e função. Diversos métodos podem ser utilizados para o tratamento. Em nosso ponto de vista, a classificação da osteoradionecrose e o tratamento descrito por MARX é um ótimo guia para a abordagem desses pacientes e suas complicações.

ABSTRACT

Introduction: The treatment of patients with Mandibular Osteoradionecrosis is challenging and almost always followed by complications. Objective: to describe the treatment of patients with mandibular osteoradionecrosis after radiation therapy to the head and neck area and its complications based on MARX's protocol. Method: Retrospective study of five patients presenting mandibular osteoradionecrosis after radiation therapy to the head and neck. They were surgically treated in Hospital Santa Catarina in the city of Blumenau - SC between the years 2004 and 2007. Results: One patient was submitted to segmental mandibulectomy followed by rigid reconstruction associated to iliac graft; three patients were submitted to a segmental mandibulectomy followed by rigid reconstruction without grafting and one patient was submitted to a marginal mandibulectomy. In two cases there was closure of the intraoral wound and absence of pain. There was an opening of the cutaneous wound with plate exhibition in three cases with large resection without grafting and microvascular reconstruction was indicated. Conclusion: The purpose of therapy of the craniofacial osteoradionecrosis is the end symptoms, the possibility of tumor recurrence and shape and functional restoration. Several methods can be used. From our point of view, the classification of the osteoradionecrosis described by MARX is a great guide for the approach of these patients and possible complications.
