Acompañando al paciente con trastorno bipolar: aportes a la psicoeducación / Accompaying the patient with bipolar disorder: a contribute the psychoeducation

ABSTRACT

Objective: Identify, from a perspective of family caretakers, elements that contribute to the psycho-education programs for bipolar patients and their families. Materials and Methods: Qualitative study, based on the theory founded. Instrument used Semi structured interview and socio demographic file. With previous informed consent, the realization of an in depth interview thirteen family caretakers of patients with bipolar disorder. The interviews were taped, codified and marked with false names in order to protect the confidentiality. The information was analyzed, and relevant aspects of the information were discussed with the participants. Results: Facing the caring for a bipolar patient generates difficulties to the family caretakers especially in relation to the elaboration of the mourning of the patient, identifying the sentiments of guilt that interfere with taking care, identify the traumatic experiences that could hinder the capacity for taking care and the fear of talking about the illness. Conclusions: It is necessary to implement programs of family psycho-education parallel to the treatment of the bipolar patient, in this way the family anxiety can be contained, preventing the psychological uneasiness in the caretakers and gain in the caretaking and auto-caretaking of the patients as well as the caregivers. The programs of psycho- education, apart from offering information should transcend towards the seeing of psychodynamic aspects of the care giving, together with offering education about the rights of the patients and the mental health system.

RESUMEN

Objetivo: Identificar, desde la vivencia de los cuidadores familiares, elementos que contribuyen a los programas de psicoeducación para pacientes bipolares y sus familias. Material y Métodos: estudio cualitativo, basado en Teoría Fundada. Instrumentos utilizados Entrevista semiestructurada y ficha sociodemográfica. Previo consentimiento informado, se entrevistaron en profundidad trece cuidadores familiares de pacientes con Trastorno Bipolar. Las entrevistas fueron grabadas, codificadas y marcadas con nombres ficticios para proteger la confidencialidad. Analizada la información, se discutió con los participantes aspectos relevantes de los datos. Resultados: Enfrentar el tratamiento de un paciente bipolar genera dificultades a los cuidadores familiares especialmente relacionadas con la elaboración del duelo sobre el paciente, sentimientos de culpa que interfieren en el cuidado, experiencias traumáticas que minan la capacidad de cuidado y el temor a hablar sobre la enfermedad. Conclusiones: Es conveniente implementar programas de psicoeducación familiar paralelamente al tratamiento del paciente bipolar, de tal manera que pueda contenerse la angustia familiar, prevenir el malestar psicológico en los cuidadores y abonar el terreno para el cuidado y el autocuidado tanto de los pacientes como de los familiares. Los programas de psicoeducación, además de ofrecer información deben trascender hacia el abordaje de aspectos psicodinámicos del cuidado, junto a ofrecer educación sobre los derechos de los pacientes y el sistema de salud mental.
