Tromboembolia de corazón derecho: manejo con trombolíticos / Thromboembolia of right heart: I handle with trombolytics

RESUMEN

Presentamos el caso de un paciente de 46 años, sin antecedentes patológicos previos, con diagnóstico de Embolia Pulmonar y Tromboembolia de corazónderecho. Fue tratado exitosamente con trombolíticos utilizando Activador del Plasminógeno Tisular Humano Recombinante (rt-PA) a dosis de 1 mg /h. A las 64 horas hubo remisión total del trombo en la aurícula derecha ysubtotal de la Embolia Pulmonar y de la trombosis de la vena ilíaca. Durante la resolución del trombo no hubo complicaciones hemodinámicas ni clínicas. El paciente fue anticoagulado con warfarina para mantener un INR mayor a 3, fue dado de alta y continua con buena evolución al tercer mes de control ambulatorio.

ABSTRACT

We present the case of a 46 year old male patient, withno previous disease, who was diagnosed with Pulmonary Embolism and Right Heart Thromboemboli. He was successfully treated with thrombolytic therapy using recombinant tissue plasminogen activator (rt-PA) at a dose of 1mg/h. After 64 hours there was total remission of the right atrial thrombus and subtotal remission of the pulmonary embolism and iliac vein thrombosis. No clinical or hemodynamic complications appeared during the resolution of the thrombus. The patient was orally anticoagulated with warfarin to maintain an INR higherthan 3, he was discharged and is doing well after three months of follow up during ambulatory care.
