Reparo de fratura radicular com tecido calcificado: relato de caso clínico / Repair of horizontal root fracture with hard tissue: case report

RESUMO

Apesar de representarem um acontecimento pouco comum, as fraturas radiculares em dentes permanentes podem reparar com a coalescência dos fragmentos, sem a necessidade de intervenção endodôntica, se o tratamento inicial adequado for prontamente instituído. Os autores relatam um caso de fratura radicular horizontal com deslocamento coronário, no qual se realizou a reposição dos fragmentos sob anestesia local. Contenção com resina composta e antibioticoterapia. Após quatro anos de acompanhamento, não foram observadas reabsorções radiculares interna ou externa, alterações do contorno do espaço periodontal, obliteração do canal radicular e sintomatologia dolorosa. Houve um aumento gradativo da radiopacidade na linha de fratura, sugerindo um processo de reparação com interposição de tecido calcificado entre os fragmentos.

ABSTRACT

In spite of representing an uncommon event, root fractures in permanent teeth can repair by the union of the fragments, without the need of endodontic intervention, if proper initial treatment is instituted quickly. The authors report a case of horizontal root fracture with extrusive luxation of the coronal fragment. The treatment consisted by tooth fragment repositioning under local anesthesia, splinting using photo polymerizable resin and antibiotic therapy. After four years of follow up, internal and external radicular resorption, alterations of the periodontal space contour and pain were not observed. There was an increase of the radiopacity on the fracture line, suggesting a repair process through the calcification between the fragments.
