RESUMEN

El gasto sanitario posee una tendencia al crecimiento descontrolado, donde los hospitales, por tornarse cada vez más quirúrgicos, acarrean con esta actividad el mayor consumo de recursos. Como se sabe, la cirugía es un proceso y la gestión de procesos es la metodología necesaria para mejorar la capacidad resolutiva y la eficiencia hospitalaria. En el campo médico el 75% del tiempo del proceso no añade valor al mismo. Esto es de particular importancia debido a su implicancia en los costos finales del producto. La búsqueda de sistemas alternativos a la hospitalización es a los fines de atender más pacientes ocupando la menor cantidad de camas. La Cirugía Ambulatoria (CA) es el modelo organizativo que resuelve este problema, permitiendo tratar al paciente correctamente seleccionado de una forma segura y efectiva, sin hacer uso de una cama de internado. El objetivo general de este trabajo es indagar sobre las principales características conceptuales y problemáticas organizacionales, administrativas y de gestión que presenta la CA, para desterrar la idea de que solo representa un ahorro económico. Todo lo contrario, se trata de un excelente ejemplo de rediseño de procesos aplicados a la satisfacción de la demanda quirúrgica, en el que el paciente obtiene un servicio de mayor calidad incluyendo la seguridad a un menor costo.

ABSTRACT

The healthcare spending is continuously growing because of the raising consume of resources due to the increasing quirurgic activity at hospitals. If we understand the surgery as a process, the managing of processes is the right methodology for improving both the resolutive capacity and the hospital efficiency. In the medical field, 75% of the process time does not add economic value to the process itself. This has particular importance for the final costs of the product.The search for alternatives to the patient hospitalization aims at offering a broader assistance to a higher number of patients while reducing the amount of occupied beds. The Ambulatory Surgery (AS) is the proper organizing model that deals with this problem, allowing the treatment of selected patients in a safe and effective way, without using an internship bed. The general aim of this work is to inquire about the main conceptual characteristics and the organizational, administrative, and managing problems of the AS, to clearly show that this kind of surgery not only represents an economical saving. On the contrary, AS is a good example of a redesign process to satisfy the quirurgic demand, in which patients obtain a higher quality service, including their safety at a lower cost.
