Laringectomía total. Análisis de las complicaciones / Total laryngectomy. Analysis of the complications

RESUMEN

Fundamento El cáncer de laringe en nuestra provincia constituye una de las causas más frecuentes de ingreso, tanto en el servicio de Urgencia, como electivamente para recibir tratamiento quirúrgico. Objetivo: Conocer la evolución postquirúrgica de los pacientes con cáncer de laringe. Método: Se realizó un estudio descriptivo y retrospectivo en el Hospital Provincial Clínico Quirúrgico «Manuel Ascunce Domenech¼ de la ciudad de Camagüey desde enero de 2003 a enero de 2005. El universo estuvo constituido por un total de 30 pacientes ingresados, a los cuales se les realizó laringectomía total. Los datos se obtuvieron de las historias clínicas, teniendo en cuenta las variables edad, sexo, antibiótico terapia y complicaciones. Resultados: Del total de pacientes laringectomizados, el 43.33 % presentó complicaciones. El grupo de pacientes comprendido entre las edades de 61 a 70 años fueron los que con mayor frecuencia se sometieron al tratamiento quirúrgico, con un 63.33 % del total, predominó el sexo masculino con 83.33 %. La complicación más frecuente fue el faringostoma que representó el 76.92 % de las complicaciones, y el 33.33 % del total de operados, seguido de las sepsis de la herida, donde prevalecieron los gérmenes gramsegún los cultivos realizados. Conclusiones: Las complicaciones del cáncer de laringe se presentan con mucha frecuencia y atentan contra el buen desenvolvimiento de estos pacientes.

ABSTRACT

Background: Larynx cancer in our province constitutes one of the most frequent causes of admission, so as in the Emergency service, as electively to receive surgical treatment. Objective: To know the post-surgical evolution of the patients with larynx cancer. Method: A retrospective and descriptive study was performed at «Manuel Ascunce Domenech¼ Surgical Clinical Provincial Hospital of Camagüey city from January 2003 to January 2005. The universe was constituted by a total of 30 patients admitted, to which a total laryngectomy was carried out. Data were obtained of the medical histories taking into account the variables age, sex, antibiotic therapy and complications. Results: Of the total of laryngectomized patients, the 43.33 % presented complications. The group of patients among the ages from 61 to 70 years was the ones that with greater frequency were submitted to surgical treatment, with a 63.33% of the total, male sex with 83.33% dominated. The most frequent complication was the pharyngostoma that represented the 76.92% of the complications, and the 33.33% of the total of the operated, followed by the sepsis of the wound, where the gramgerms prevailed according to the cultures carried out. Conclusions: The complications of larynx cancer are presented with a lot of frequency and it attempted against the good development of these patients.
