Intoxicación accidental con mercurio elemental / Accidental elemental mercury poisoning

RESUMEN

Paciente mujer de 19 años procedente de Cusco - Perú, que había ingerido tres meses atrás café con mercurio elemental en forma accidental; es transferida al Hospital Nacional Dos de Mayo ingresando por Emergencia con sintomatología múltiple y progresiva cefalea, hemoptisis, disnea, cambios conductuales como irritabilidad y tristeza, sabor metálico y parestesias en miembros inferiores. El síntoma predominante fue la disnea a medianos y pequeños esfuerzos. Se determinó mercurio sérico (178,5 ugr/l) y urinario (465 ugr/l) encontrándose los niveles elevados por encima del valor de referencia para la población no expuesta. Fue tratada con penicilamina 250 mg cada 6 horas durante 10 días. Se adicionó a la terapia Piridoxina 100 ugr/ día debido al riesgo de convulsiones por tratamiento con penicilamina. La evolución fue favorable, sin complicaciones y con escasos efectos secundarios posterior a la terapia quelante.

ABSTRACT

Patient 19-year-old woman from Cuzco - Peru, who had ingested coffee with elemental mercury accidentally three months ago, is transferred to the Dos de Mayo National Hospital, where she is admitted to emergency room, with multiple and progressive symptoms headache, hemoptysis, progressive dyspnea, behavior changes such as irritability and sadness, metallic taste and paresthesias in lower limbs. The predominant symptom was progressive dyspnea in medium and small efforts. Mercury levels were determined in serum (178,5 ugr/l) and urine (465 ugr/l), being elevated above the reference value for the unexposed population. She was treated with penicillamine 250 mg every 6 hours for 10 days. Additionally, she was treated with pyridoxine 100 ugr / day, because of the risk of seizures by treatment with penicillamine. The evolution was favorable, without complications and with few side effects with chelating therapy.
