Tuberculosis pancreática fistulizante asociada con várices gástricas sangrante por hipertensión portal segmentaria / Fistulizing pancreatic tuberculosis associated to bleeding gastric varices due to segmental portal hypertension

RESUMEN

Varón de 27 años, no inmunosuprimido, que ingresó al hospital por hemorragia digestiva alta secundaria a varices gástricas de localización fúndica y dolor abdominal. Luego de los estudios, se diagnosticó tuberculosis de la cola del páncreas y compresión del hilio esplénico con hipertensión portal segmentaria, lo que se manifestó como una hemorragia digestiva alta varicosa. También, presentó una fístula cutánea a la región lumbar izquierda y extensión al músculo psoas iliaco, que compromete tres vértebras lumbares. Con el tratamiento antituberculoso evolucionó favorablemente. Después de un seguimiento de nueve meses el paciente estuvo asintomático.

ABSTRACT

A 27-year-old male patient entered to the hospital because of upper gastrointestinal bleeding secondary to fundic gastric varices and abdominal pain. After a thorough study, he was diagnosed of tuberculosis of the tail of the pancreas and compression of the splenic hilum with segmental portal hypertension, which manifested itself as a variceal upper gastrointestinal bleeding. Also, he developed a cutaneous fistula to the left lumbar region, and extension to the iliac psoas muscle, involving three lumbar vertebrae. With the TB treatment, the patient evolved favorably. After a nine-month follow-up, the patient was asymptomatic.
