¿Son útiles los opioides y adyuvantes en el dolor agudo? / Are necessary the opioids and the adjuvants in the acute pain?

RESUMEN

Introducción. El abordaje terapéutico del dolor agudo no debe ni puede hacerse desde una sola perspectiva y es por eso que se utilizan fármacos analgésicos como opioides y otros que contribuyen de forma eficaz al alivio del dolor. Objetivo. Realizar una puesta al día sobre la actualidad de los opioides y adyuvantes en el tratamiento del dolor agudo y crónico. Desarrollo. Se realizó una actualización del tema sobre el papel de los opioides en el tratamiento del dolor, así como las ventajas y desventajas de otros fármacos adyuvantes como los antidepresivos y anticonvulsivantes. Se hace énfasis en las ventajas de explorar las combinaciones de opioides para mejorar su eficacia. Los antidepresivos tricíclicos se utilizan en el dolor neuropático en particular y en el dolor crónico en general, aunque el paciente no sufra depresión o ansiedad. Para estos tratamientos se emplean dosis menores y a menudo producen efectos más rápidos que otros medicamentos. Por su parte, los nuevos antiepilépticos han aportado beneficios considerables por las características farmacocinéticas y farmacodinámicas de este grupo farmacológico. Hoy, se conoce mejor sus mecanismos de acción y sus interacciones lo que ha potenciado la utilización de los fármacos antiepilépticos en entidades clínicas ajenas a la epilepsia, como es el caso de su uso en el tratamiento del dolor. Conclusiones. Se confirma la utilidad de los opioides como analgésicos asociados a antidepresivos, anticonvulsivantes y antiepilépticos en el tratamiento del dolor agudo.

ABSTRACT

Introduction: The therapeutic approach of acute pain must not to be and not to made from one only perspective and therefore the use of analgesic drugs like opioids and other types to achieve an effective relief of pain. Objective: To make a updating on the opioids and adjuvants in the treatment of chronic and acute pain. Development Authors made an updating on this subject related to the role of opioids in the treatment of pain, as well as the advantages and disadvantages of other adjuvant drugs like the antidepressants and the anticonvulsants. It is emphasized on the advantages of exploration of the combination of opioids to improve its effectiveness. The tricyclic antidepressants are used in the neuropathic pain in particular and in the chronic pain in general, although the patient has not depression or anxiety. For these treatments lower doses are used and often produce more quick effects than other drugs. Besides, the new antiepileptic drugs have offered significant benefits due to its pharmacokinetic and pharmacodynamic characteristics of this pharmacological group. Today, there is a great knowledge on its mechanisms of action and its interactions increasing the use of the antiepileptic drugs in clinical entities no related to epilepsy by example, in the case of treatment of pain. Conclusions: The usefulness of opioids is confirmed as analgesic drugs associated with antidepressants, anticonvulsants and antiepileptics in the treatment of acute pain.
