Habilidades clínicas complejas en estudiantes de enfermería / Complex clinical skills in nursing students

RESUMEN

Introducción: el curso de cuidados intensivos del Instituto Mexicano del Seguro Social está diseñado en dos etapas (teórica y práctica) mediante las cuales se pretende formar a enfermeras (os) con conocimiento y habilidades que les permitan brindar atención de enfermería a pacientes en estado crítico. El objetivo de este estudio es identificar el grado de desarrollo de las habilidades clínicas complejas en los estudiantes de enfermería de cuidados intensivos. Material y métodos: estudio prospectivo, comparativo y longitudinal realizado en 1999; población constituida por tres grupos de alumnos, en total fueron 57. Se construyó un instrumento de 123 ítems bajo la perspectiva de aprendizaje participativo que exploró tres indicadores de la aptitud clínica aplicada en tres momentos ­inicio, mitad y fin de la práctica clínica­; se utilizaron pruebas de Kruskal Wallis, de rangos Wilcoxon y X2. Resultados: los grados de dominio de la aptitud clínica de los alumnos en las mediciones inicial y final se ubicaron en bajo y muy bajo 44% y 33% respectivamente. Discusión: en la integración de teoría con práctica el alumno desarrolla un proceso educativo. Se llevó a cabo la construcción de un instrumento distinto que exploró algo más que el recuerdo siendo relevante al tratar de acercar al alumno a situaciones reales en las que se encuentre durante su práctica profesional.

ABSTRACT

Introduction: the intensive care nursing course is development of complex clinic skills in students. The objective is identify the development of certain degree of complex clinic ability, in the clinical practice of intensive care nursing course. The intensive care nursing course has two steps: theoretical and practice. The first one has as proposal to get the curriculum contents. In the second the students develop skills for critical ill patient whole care. Due to lack of interaction with the patient since the beginning of the course, at the end of process students aren't reflexioning about critical care patient and its care. Material and method: longitudinal study, 57 students population from three groups, under the taking-part learning perspective a123 questions instrument was built, they all explore three clinical ability indicators, it was applied in three moments beginning, middle and finish of the clinical practice. Outcome shows that clinical practice get a low complex clinic ability development. Results: the grade of domains of clinical skills in the student were at the begin and at the end with in the scale low and very low with 44% and 33% respectivily. Discussion: the authors make stand out of a different instrument exploring something else that recall and try to face students with real situation like those they will find in their professional practice.
