The transseptosphenoidal approach to the hypophysis is the one preferred for certain cituations, and its use represents great advantages. Previous septal surgery complicates the usage of this approach. The authors describe the techniques they have used to overcome this trouble, remarking the one they are using nowadays which is their primary selection for solving the difficulties caused by a previous septal surgical intervention
