Úlcera gástrica gigante por citomegalovirus en infección VIH/SIDA / Giant gastric ulcer by cytomegalovirus in infection VIH/SIDA

RESUMEN

La enfermedad por citomegalovirus (CMV) es una importante causa de morbilidad en inmunosuprimidos por el Virus de la Inmunodeficiencia Adquirida (VIH). Presentamos a un hombre de 43 años con insuficiencia renal, en hemodiálisis, con varias transfusiones por anemia, que presentó una enfermedad de tres meses, caracterizada por epigastralgia a predominio nocturno que calmaba con antiácidos. Conjuntamente presento llenura, vómitos y baja de peso (18 Kg). Al examen palidez, adenopatías en cuello y región inguinal, epigastrio doloroso a la palpación profunda. Los exámenes mas destacados fueron Hb10mg/ml, VCM83.5, Leucocitos 7000, Linfocitos 1715, eritrosedimentación 49mm/h, Thevenonnegativo ( heces) y la presencia de Giardia lamblia en heces. El estudio para Elisa-HIV y el Western-Blot fueron positivos. IgG para CMV y Herpes (I y II) positivos; el IgM para los mismos negativo. La endoscopia mostró al esófago tapizado por placas blanquecinas compatibles con candidiasis; a nivel del cuerpo gástrico se observó una grande lesión ulcerada (45 x 21 mm), profunda, bordes definidos y lecho blanquecino. La biopsia mostró fondo y borde de úlcera con células citomegálicas con inclusiones virales intranucleares y citoplasmáticas, con inmunohistoquímica se confirmó la presencia del CMV. Recibió ganciclovir mejorando, luego inició terapia antirretroviral (TARGA) contra el VIH.

ABSTRACT

Cytomegalovirus infection is an important cause of morbidity in immunosupressed patients with Human Immunodeficiency Virus (HIV). In this paper we present a 43 years old manwith renal failure under hemodialysis, several blood transfusions because of anemia and three months of disease characterized by epigastric pain, specially at nights, ameliorated with antacid drugs. Other symptoms were early satisfy, vomits and weigh loss (18Kg). At clinical exam, the patient was pallid, presented adenopathies at cervical and inguinal regions and had a pain at epigastric region in profound touch palpation. The most importantexams were HB 10mg/dl, CMV 83.5, leukocytes 7000, lymphocytes 1715, erythrocyte sedimentation rate 49mm/h, thevenon test (-), and Giardia lamblia trophozoites in stools. The studies demonstrated the patient was seropositive for HIV and the tests for IgG CMV and IgG Herpes virus resulted seropositives too. At endoscopy the esophagus mucosa was covered by a white plaque which suggests candida infection. In the stomach, over the bodygastric, we found a big and deep ulcerated lesion (45x41mm), with defined rims and white fund. Biopsy from the edges of the gastric ulcer had the characteristic CMV intranuclearand intracytoplasmic inclusions; we confirmed the diagnosis by immunohystochemistry. The patient receives ganciclovir an then HAART and is getting well.
