RESUMO
A angina de Ludwig, uma celulite originada de infecção dentária em mais de 90% dos casos, é ainda uma ameaça à vida do paciente, pelo seu risco iminente de obstrução das vias aéreas. Caracteriza-se clinicamentepor um aumento volumétrico eritematoso nos espaços submandibular, sublingual e submentoniano.O sucesso do tratamento é baseado no seu reconhecimento precoce, uso de antibióticos adequados, manutençãodas vias aérea e a drenagem do foco infeccioso. O presente trabalho tem como objetivo apresentardois casos clínicos de angina de Ludwig em pacientes debilitados sistemicamente, ressaltando o potencialde letalidade dessa enfermidade. Os tratamentos foram baseados no diagnóstico precoce, administraçãoparenteral de antibióticos apropriados, manutenção das vias aéreas, drenagem cirúrgica e remoção do focoinfeccioso. A conduta terapêutica instituída promoveu o controle da infecção e evitou maiores complicações.Portanto, conclui-se que é de extrema importância a realização do diagnóstico precoce e a intervenção imediataadequada, incluindo o controle dos fatores de risco envolvidos.
ABSTRACT
The angina of Ludwig, an originated celulite of dental infection in more than 90% of the cases, is still a threat to the lifeof the patient, for its imminent risk of obstruction of the airways. It is clinically characterized for volumetric increase insubmandibular, sublingual and submentoniano facial spaces. The success of the treatment is based on rapid diagnosis,antibiotic terapy, maintenance of the airways and the infectious focus drainage. The present work has as objective topresent two clinical cases of Ludwig´s angina in systemic problems patients, standing out potentially life-threatening disease.The treatments had been based on the early diagnosis, appropriate antibiotic administration, airways management,surgical drainage and infectious focus removal. The instituted therapeutics promoted the infection control and preventedcomplications. Thus, the accomplishment of the early diagnosis and the immediate intervention are very important,including the factors of risk control.
