RESUMEN
Una sonda de alimentación colocada a través de una gastrostomía endoscópica percutánea (GEP) es un método seguro y efectivo para ofrecer nutrición enteral prolongada en pacientes que no pueden deglutir. Revisamos las indicaciones de dicha técnica y nuestra experiencia de 7 años en un centro privado gastroenterológico en 103 pacientes y 128 colocaciones tipo "pull" o tracción. Todos los pacientes recibían vitamina k y Cefalotina EV previo al examen. Tuvimos 1.56 por ciento de complicaciones mayores, ambas por reflujo y aspiración a 48 y 72 horas con mortalidad directa. La complicación menor más común fue la infección del ostoma en 8.9 por ciento. No hubo mortalidad adicional ni durante la realización del examen ni otras por el procedimiento hasta los 30 dias. El total de nuestros pacientes tenían indicación por cuadro neurológico siendo las formas más comunes la demencia post infarto y accidente cerebro vascular con 23.3 por ciento y 21.4 por ciento. La gastrostomía endoscópica percutánea se ha convertido en el método de elección para facilitar una nutrición enteral prolongada.
ABSTRACT
Percutaneous endoscopic gastrostomy tube feeding is a safe and effective method of providing long-term enteral nutrition in patients who are unable to swallow and offers important advantages over nasogastric tub feeding. The indications of this procedure have been received and over 7 years of practice on 103 patients and 128 procedures have been experienced with Ponsky pull techniques. All patients were given vitamin K and a prophylactic antibiotic (chephalotine) prior to testing.1.56 per cent of major deadly complications have been reportes both for reflux and aspiration at 48 and 72 hours post treatment. The most common minor complication was wound infection in 8.9 per cent of patients. There was no further mortality during testing or while undergoing treatment in the following30 days. All patients were suffering from post heart attack dementia and stroke (CVA) at a rate of 23.3 per cent and 21.4 per cent, respectively. Percutaneous endoscopic gastrostomy has become the method of choice to sustain long-term enteral nutrition.
