RESUMO
A síndrome da apneia obstrutiva do sono (SAOS) é muito prevalente e ainda pouco reconhecida na população em geral. Entre os pacientes com hipertensão arterial sistêmica (HAS), a SAOS é encontrada em mais de um terço dos pacientes. Existem evidências crescentes de que a SAOS e a HAS frequentemente coexistem, não somente por terem fatores de risco comuns (obesidade, sedentarismo e idade) mas também porque a SAOS contribui para a gênese da HAS. A SAOS causa alterações hemodinâmicas e metabólicas não somente durante o sono, mas que persistem ao longo das 24 horas. Nesta breve revisão, serão discutidos os princípios fisiopatológicos envolvidos na síndrome da apneia obstrutiva do sono e sua interação com a HAS, como também as peculiaridades do seu tratamento com base nas mais recentes evidências de estudos clínicos.
ABSTRACT
The syndrome of obstructive sleep apnea (OSA) is very prevalent and little recognized in the general population. Among patients with hypertension (HBP) OSAS is foundin more than a third part of patients. There is increasing evidence that OSA and hypertension often co-exist not only because they have common risk factors (obesity, sedentary lifestyle and age), but because OSA contributes to the genesis of hypertension. OSA causes hemodynamic and metabolic changes not only during sleep, but still over 24 hours. We discuss in this brief review the pathophysiologic principles involved in the syndrome of obstructive sleep apnea and its interaction with the HBP, but also the peculiarities of its treatment based on the most recent evidence of clinical studies.
