RESUMO
A proteína C reativa de alta sensibilidade vem sendo proposta como um biomarcador de desfechos cardiovasculares desde a década passada, quando foram descritos os primeiros estudos observacionais sugerindo um promissor valor preditivo, tanto para desfechos coronários como para acidente vascular cerebral isquêmico. Em paralelo, cresceram as evidencias de que a aterosclerose, o substrato comum dessas doenças cardiovasculares, seja um contínuo processo inflamatório vascular. Dessa forma, a busca por biomarcadores inflamatórios tornou-se foco de grande interesse, determinando a realização de importantes estudos clínicos e experimentais. Nois dois últimos anos, o papel da proteína C-reativa foi mais firmemente estabelecido com base no estudo JUPITER e em meta-análise para desfechos cardiovasculares e mortalidade total. De fato, a utilização de uma estatina efetiva nesses pacientes com valores relativamente normais de colesterol de lipoproteínas de baixa densidade (LDL-colesterol), mas com níveis elevados de proteína C-reativa de alta sensibilidade...
ABSTRACT
Highly-sensitive C-reactive protein has been proposed as a marker of cardiovascular events since the last decade, when the first observational studies showed its promising predictive value for both coronary events and ischemic stroke. Meanwhile, there has been increasing evidences that atherosclerosis, the common aspect of these cardiovascular diseases, is a continuous inflammatory process. Therefore, the search for inflammatory biomarkers has become an important focus of interest, leading to the conduction of important clinical and experimental investigations. In the last two years, the role of C-reactive protein was firmly established on basis of the JUPITER trial and in recent meta-analysis for cardiovascular events and total mortality. In fact, the use of effective statin on patients with relatively normal low-density lipoprotein cholesterol levels (LDL-cholesterol), but with high levels of C-reactive protein was associated with a substantial decrease in cardiovascular events...
