RESUMEN
Con el presente reporte nos proponemos exponer las experiencias del equipo de trabajo en cuanto al diagnóstico y tratamiento de esta infrecuente entidad tumoral, aportando un nuevo caso a la literatura de rabdomiosarcoma de vejiga urinaria. Se presenta un paciente masculino de 48 años de edad que acudió a consulta de Cirugía General, remitido desde su área de salud en el nivel primario de atención, por aumento de volumen en hemiabdomen inferior y disuria. Se diagnosticó un tumor intrabdominal. Decidimos tratamiento quirúrgico mediante laparotomía, y se evidenció tumoración que correspondía a la vejiga urinaria, de base ancha, ocupando la parte posterior y lateral derecha del órgano, de color blanquecino y consistencia blanda, que no guardaba relación con órganos vecinos; los restantes órganos se encontraron indemnes. Identificamos ambos meatos ureterales, practicando exéresis del tumor. Se deja una sonda Foley Núm. 20 por cistostomía, cerramos vejiga en dos planos. Se demostró histológicamente que la tumoración correspondía con un rabdomiosarcoma pleomórfico. Se siguió su evolución interconsultándose con el Servicio de Oncología para el tratamiento posterior.
ABSTRACT
With the present report we propose ourselves to expose the experiences of the equipment of job as for the diagnosis and processing of a tumour little frequent, contributing a new case to the literature of rhabdomyosarcoma of urinary bladder. This is a case of a 48-year-old man who came to General Surgery office due to an increase of volume in the abdomen and disuria. He was admitted and diagnosed an intrabdominal giant tumour. It was decided to perform a surgical treatment by means of a laparotomy. During surgery, it was observed that the tumour corresponded to the urinary bladder. The operation continued and a big tumour of wide base was found occupying the posterior and lateral part of the urinary bladder, with whitish colour and a little soft consistency. The right and left ureteral meatus was catheterized, and exeresis of the tumour was performed. A Foley catheter no. 20 was placed in right and left position by cystostomy. The urinary bladder was closed in 2 planes, and the abdominal cavity was checked. In the histological study, it was proved that the tumour was a polymorphic rhabdomyosarcoma. Its evolution was followed and interconsultations were made with the oncology service for further treatment.
