RESUMO
As fissuras craniofaciais constituem anomalias congênitas raras. Localizam-se em determinadas linhas através das pálpebras, sobrancelhas, narinas, lábios e maxilas, atingindo partes moles e ósseas. São de etiologia ainda desconhecida. A classificação mais utilizada para as fissuras raras de face é a de Tessier. É baseada em relações anatômicas obtidas em observações clínicas e achados pós-operatórios. As fissuras números 0 e 14 situam-se na linha média do crânio e da face. Podem-se apresentar como uma fissura com as características de excesso de tecido ou agenesia dos mesmos. O correto diagnóstico é fundamental para abordagem cirúrgica. O tratamento é baseado em sutura dos tecidos moles, em três planos cutâneo, muscular e mucoso; zetaplastias; tratamento ortodôntico; enxertos ósseos, quando necessário. Os autores descrevem o caso de um paciente de 2 anos e 3 meses, sexo masculino, apresentando fissuras 0 e 14 de Tessier. A primeira abordagem baseou-se no tratamento do defeito das partes moles dessas fissuras, sendo realizada em dois tempos cirúrgicos.
ABSTRACT
The clefts craniofacial constitute congenital rare anomalies. They are located in determined lines through the eyelids, eyebrows, nostrils, lips and jawbones, reaching soft and bone parts. They are of unknown etiology. The classification most used for the rare clefts of face is it of Tessier. It is based on anatomical relations obtained in clinical observations and post-operative finds. The clefts numbers 0 and 14, positions himself in the middle line of the skull and of the face. Can be present the characteristics is excess of tissue or the lack of the same. The correct diagnose is essential for the surgical approach. The treatment is based on suture of the soft cloths, on three plans cutaneous, muscular and mucous; Zplasty; treatment tooth; bone grafts, when necessary. The authors describe the case of a 2-year-old patient and 3 months, masculine, 0 and 14 showing cracks of Tessier. The first correction was based on the treatment of soft tissue defect of these cracks, planned in two surgical times.
