Apendicitis con aire intraluminal / Appendicitis with intraluminal air

RESUMEN

La presentación de apendicitis aguda con aire intraluminal es un hallazgo poco frecuente. La ecografía es un método diagnóstico ampliamente utilizado y aceptado para establecer el diagnóstico de apendicitis; sin embargo, su desempeño se ve limitado en situaciones en las que existe interposición de aire. El apéndice inflamado que presenta distensión de su luz por gas puede ser fuente de error en la ecografía, al establecerse un diagnóstico falso negativo. En este artículo se presentan los casos de tres pacientes con apendicitis, en quienes la interposición de aire intraluminal ocultó su diagnóstico por ecografía. En los tres casos el diagnóstico definitivo se estableció mediante escanografía y confirmación quirúrgica.

ABSTRACT

The presentation of acute appendicitis with intraluminal air is an uncommonpresentation. Ultrasound is a widely used and accepted to establish the diagnosis of appendicitis, but its performance is limited in situations where there is interposition of air. Inflamed appendix which presents with distention by gas can be source of errorin ultrasound to establish a false negative diagnosis. In this article we present three patients with appendicitis where the interposition of intraluminal air hid their diagnostic by ultrasound. In all three cases the definitive diagnosis was established by computed tomography and surgical confirmation.
