RESUMEN

La Hernia Diafragmática Congénita (HDC) es una malformación que resulta de la fusión incompleta de la membrana pleuroperitoneal; ocurre en alrededor de 1/2000-5000 recién nacidos vivos. Se presenta generalmente como un cuadro de distress respiratorio en el período neonatal con morbimortalidad importante, debido a grados variables de hipoplasia e hipertensión pulmonar. El tratamiento se basa en mantener las mejores condiciones respiratorias en el niño mediante su manejo en terapia intensiva neonatal y, una vez que se logra este objetivo, se debe proceder con la corrección quirúrgica del defecto anatómico. Desafortunadamente, a pesar del avance en el manejo respiratorio del recién nacido gravemente enfermo, la mortalidad por este padecimiento se reporta por arriba de 75. Presentamos el caso de un paciente que desde el nacimiento debuta con datos de dificultad ventilatoria progresiva a quien se le diagnostica una hernia diafragmática congénita mediante radiografía de tórax; además se revisa la patogenia, tratamiento y pronóstico de la enfermedad.

ABSTRACT

The congenital diaphragmatic hernia (CDH) is a malformation which is due to the incomplete fusion for pleuroperitoneal membrane. It occurs in about 1/2000-5000 living newborns. The case generally occurs as a manifestation of respiratory distress in neonatal period with an important morbidity, due to variable degrees of hypoplasia and pulmonary hypertension. The treatment consists in maintaining the best respiratory conditions in the offspring through its treatment in the neonatal intensive care and, once this objective is achieved, the surgical correction can be performed. Unfortunately, despite the advance in the respiratory treatment of the newborn seriously sick, morbidity for this suffering was reported above 75. We are presenting the case of a patient who has a diagnosis of progressive ventilation difficulty from birth. He was diagnosed a congenital diaphragmatic hernia of Thorax; Furthermore the pathogenesis, treatment and prognosis of the illness were checked.
