RESUMEN

Sin olvidar que la leche humana es el mejor alimento para el bebé, este texto tiene como propósito opinar sobre el funcionamiento del lactario en el contexto hospitalario y describir cómo se articula su existencia desde la política de promoción, protección y apoyo a la lactancia materna. Desde la implementación de las acciones mundiales para motivar e incrementar la lactancia, el lactario ha sido cuestionado por favorecer el uso de sucedáneos y no ajustarse a la iniciativa "Instituciones amigas de la mujer y la infancia" de la UNICEF. Su función principal es la producción de fórmulas lácteas, calculadas a partir de las necesidades propias de consumo de la población infantil, pero normativamente los hospitales que cuentan con servicios de obstetricia y pediatría, deben tenerlo porque el proceso de habilitación institucional lo obliga; además, las situaciones médicas en donde la lactancia está contraindicada o motivos particulares como la muerte materna, entre otros, hacen necesario ofrecer fórmulas infantiles y más cuando no se cuenta con un banco de leche humana. Otro agravante adicional, es que la cultura de donación de leche humana no está sensibilizada socialmente y las familias beneficiarias la rechazan a pesar de la motivación del personal de salud y la evidencia científica. Desde lo administrativo, los recursos para implementar los bancos no son sostenibles dado que se requiere una gran infraestructura para asegurar la inocuidad de este producto. Estas circunstancias, claro está, llevan a desviar las acciones orientadoras de la política en salud y no lograr las metas programadas.

ABSTRACT

Without forgetting that human milk is the best food for babies, this text aims to review the operation of lactaria in the hospital setting and describe how it articulates its existence from the policy of promoting, protecting and supporting breastfeeding. Since the implementation of global actions to encourage and increase lactation, has been questioned by lactaria favor the use of substitutes to fit on the initiative "Institutions friends of women and children" from UNICEF. Its main function is the production of infant formula, calculated from the consumption needs of the child population, but a regulation, the hospitals with obstetrics and pediatrics, should have it because the process of enabling institutional forces him, in addition, medical situations where breastfeeding is contraindicated or personal reasons such as maternal death, among others, are necessary to provide infant formula and when you do not have a human milk bank. Another additional aggravating is that the culture of human milk donation is not sensitized socially and beneficiary families reject despite the motivation of health and scientific evidence. From the administrative resources to implement the banks are not sustainable because it requires a large infrastructure to ensure the safety of this product. These circumstances, of course, carry forward the guiding policy actions in health and achieve program goals.
