RESUMO
A doença periodontal é uma doença imunoinflamatória que acomete os tecidos de suporte do dente (gengiva, cemento, ligamento periodontal e osso alveolar) em resposta ao antígeno bacteriano do biofilme, por isso, este é um fator etiológico para doença periodontal. Porém, alguns fatores ambientais e sistêmicos já estão comprovadamente envolvidos na progressão da doença como fumo e Diabetes Mellitus (DM). A DM representa um grupo de doenças metabólicas caracterizadas por hiperglicemia, sendo causada por deficiência total ou parcial da produção de insulina. Outras alterações que ocorrem no paciente com DM são a disfunção de oxigênio, transporte de nutrientes e disfunção de anticorpos, desencadeando, assim, um fisiológico que aumenta a susceptibilidade à doença periodontal. A duração e magnitude da hiperglicemia estão fortemente relacionadas com doenças microvasculares causando deficiências na resposta celular imunológica do indivíduo portador de DM. Portanto, esta revisão de literatura, visa correlacionar a resposta da microcirculação do periodonto com a DM. Apesar de não existirem estudos que concretizem a existência de um envolvimento microvascular no periodonto, já foi comprovado que ambas as doenças (DM e periodontite) possuem um envolvimento bidirecional, no qual a manutenção de uma auxilia no controle da outra.
ABSTRACT
The periodontal disease is an immune inflammatory one afflicting the tissues that support the tooth (gum, cement, periodontal ligament and alveolar bone) as a response to the bacterial antigen of the biofilm, hence being a causative agent for the periodontal disease. However, it has been proved that some environmental and systemic factors are involved in the progression of the disease, as well as smoking and Diabetes Mellitus (DM). DM is a set of metabolic diseases which are characterized by hyperglycemia, which is caused by the total or partial deficiency in the production of insulin. The length and strength of the hyperglycemia are directly related to microvascular diseases, causing deficiencies in the immunological cell response of the DM bearer. Thus, we conducted this literature review, aiming to correlate the response of periodontal microcirculation with DM. Even though studies yet do not show that there is also a microvascular involvement in the periodontium, it has already been proved that both diseases (DM and Periodontitis) have a two-way involvement, where the maintenance of one assists in the control of the other.
