Comparison of two therapies in treatment of elderly unstable in-tertrochanteric fractures / 中国现代医生

ABSTRACT
Objective To compare the clinical efficacy of hemiarthroplastyand and proximal femoral intramedullary nailing treatment (PFNA) in treatment of elderly unstable intertrochanteric fracture. Methods A total of 368 cases of unstable intertrochanteric fracture were selected in our hospital. They were divided intoⅠgroup (184 cases) andII group (184 cases) by using the method to random number table. TheⅠgroup was treated with hemiarthroplasty, while the IIgroup was treated with PFNA. Results The operative time, postoperative and postoperative bleeding, operation of drainage, the incidence of complications inIIgroup were significantly lower than those inⅠgroup, the differences were statistically significant(t=-16.58,-9.38,-7.26,χ2=9.125, P<0.01). The limb weight bearing time, excellent and good rate of hip function and late complication incidence rate inⅠgroup were better than those of IIgroup. There were statistical differences between two groups (Z=11.11,P<0.05). Conclusion Hemiarthroplasty is suitable for patients who have good body condition, while there is a high early complication incidence rate. Those patients who have poorly body condition and more basic diseases, PFNA would be a better choice, while more attention should be paid to later complication in-cidence rate.
