Por intermédio da vida cotidiana de doentes com insuficiência renal crônica em hemodiálise: entre o inevitável e o casual / Dialectics of patients' daily life with chronic renal failure in hemodialysis: between the unavoidable and the casual

RESUMO

Por intermédio deste estudo procurou-se compreender a realidade social do cotidiano da vida dos doentes com insuficiência renal crônica em hemodiálise da Unidade de Nefrologia da Irmandade da Santa Casa de Misericórdia de Fernandópolis. Pretendeu-se entender este fenômeno sob a visão do referencial teórico-metodológico do materialismo histórico e dialético. As entrevistas realizadas com 18 doentes foram submetidas ao procedimento de análise de discurso, revelando a dialética da vida cotidiana destes doentes, representadas pelos temas concepções sobre o processo saúde-doença; o tratamento por intermédio da hemodiálise e suas conseqüências; o papel da equipe de saúde no tratamento hemodialítico e as perspectivas de vida dos doentes. Estes temas foram, então, analisados frente as categorias processo saúde-doença; possibilidade x realidade e necessidade x casualidade. Para estes doentes o tratamento hemodialítico é inevitável e o transplante é casual, assim, entre esta relação dialética está a enfermagem que precisa ampliar sua compreensão sobre a árdua, triste, difícil e monótona realidade e suas possibilidades de transformação. É necessário que a enfermagem reconheça essa realidade e transforme sua prática profissional de modo ético e consciente, exercitando com os doentes sua cidadania e valendo-se dela para uma melhoria na qualidade da assistência prestada.

ABSTRACT

The objective of this study was to understand the social reality of patients´ daily life with chronic renal failure in hemodialysis at the Nephrology Unit of the "Irmandade da Santa Casa de Misericordia", in Fernandópolis. To understand this phenomenon from the theoretical-methodological referential of the historic and dialectc materialism. The interviews were performed with 18 patients submitted to analysis of speech procedure, revealing dialectics of theses patients´ daily life and were represented by the following subjects: conceptions on the health-disease process; treatment by means of hemodialysis and the perspectives of the patients´ life. These subjects were then analyzed regarding the categories health-disease process; possibility x reality and need x casualness. These patients, considered hemodialysis as a treatment unavoidable and the transplant casual, thus, between this dialectic relationship there is the nursing which needs to extend its comprehension on the arduous, sad, difficult and monotonous reality and the possibilities of transformation. Nursing must recognize this reality and to ethically and consciously transform its professional practice, exercising with the patients its citizenship and improving the quality of the assistance giving.
