A supervisão clínico-institucional em análise: a narrativa de uma experiência / Clinical-institutional supervision under analysis: the narrative of an experience

RESUMO

Esta dissertação teve como objeto a dinâmica da supervisão clínico-institucional (SCI) como espaço de educação permanente e produtor de saberes e de mudança das práticas em Saúde Mental. Entende-se que esta constitui um dos dispositivos de mudanças na gestão do trabalho em Saúde Mental, com a função de fortalecer a Reforma Psiquiátrica Brasileira no que tange à proposta de novas formas no cuidado ao portador de sofrimento psíquico. Através do percurso da pesquisadora em alguns serviços de saúde mental, relata perceber que podem apresentar um viés de improdutividade, aniquilamento e constrangimentos que provocam um distanciamento dos saberes que sustentam o espaço. Os objetivos da pesquisa foram mapear os saberes que sustentam a supervisão clínico-institucional e cartografar como esses saberes operam na construção dessa Supervisão. Metodologia: o presente estudo realiza uma abordagem cartográfica, se constituindo uma pesquisa-intervenção, com abordagem qualitativa. Os participantes foram os profissionais de saúde e a supervisora de um Centro de Atenção Psicossocial III, do município do Rio de Janeiro. Os dados foram produzidos em cinco encontros e tiveram como base as narrativas dos participantes e o instrumento de coleta foi o diário de campo. Discussão as narrativas foram transcritas e a partir delas se revelam dois marcadores supervisão e implicações da supervisora. Os resultados apontaram que vários saberes sustentam a supervisão clínico-institucional, sendo eles Psicologia, Enfermagem, Medicina, Serviço Social, Musicoterapia, Artes, Dança (expressão corporal), Gestão, Política, e a concepção e funcionamento do território. Esses saberes, na maioria das vezes, dialogam e possibilitam a produção do cuidado no serviço e no território, a articulação de serviços de saúde, o compartilhamento do cuidado, o trabalho intersetorial e o acolhimento à crise e ao sofrimento psíquico. Como considerações finais, a pesquisa conclui que asupervisão, neste serviço, opera contemplando os seus pressupostos, em que a educação permanente e a produção do cuidado em Saúde Mental são fomentados e realizados.

ABSTRACT

The object of this thesis is the dynamics of clinical-institutional supervision (SCI) as a space for permanent education and producer of knowledge and change of practices in Mental Health. It is understood that this constitutes one of the devices for changes in the management of work in Mental Health, with the function of strengthening the Brazilian Psychiatric Reform with regard to the proposal of new ways in the care of patients with psychological distress. Through the researcher's path in some mental health services, she reports realizing that they may present a bias of unproductiveness, annihilation, and constraints that provoke a distance from the knowledge that sustains the space. The objectives of the research were to map the knowledge that supports clinical-institutional supervision and to map how that knowledge operates in the construction of this Supervision. Methodology: the present study takes a cartographic approach, as intervention research with a qualitative approach. The participants were health professionals and the supervisor of a Psychosocial Care Center III, in the city of Rio de Janeiro. The data were produced in five meetings and were based on the participants' narratives and the collection instrument was the field diary. Discussion: the narratives were transcribed and, from them, two markers are revealed supervision and implications of the supervisor. The results showed that several types of knowledge support clinical-institutional supervision, namely Psychology, Nursing, Medicine, Social Work, Music Therapy, Arts, Dance (body expression), Management, Politics, and the conception and functioning of the territory. This knowledge, in most cases, dialogues and enables the production of care in the service and in the territory, the articulation of health services, the sharing of care, intersectoral work, and the acceptance of crisis and psychological suffering. As final considerations, the research concludes that supervision, in this service, operates considering its assumptions, in which permanent education and the production of mental health care are promoted and carried out.
